University of Rhode Island Kingston, RI 02881 ZEISS LSM 700 Class IIIB LASER Safety and Hazard Acknowledgment This is to acknowledge that I have read the referenced information listed below and that I am aware of and understand the hazards associated with the laser components of the Zeiss LSM 700 scanning confocal module attached to the Zeiss AxioImager microscope. I also acknowledge that I have read and agree to comply with the general rules for safe conduct in this laboratory and that I have received training in the safe operation of the Zeiss AxioImager light microscope and the Zeiss LSM 700 scanning confocal module by the RIGSC manager. Comments & Warnings: 1. The Zeiss AxioImager light microscope and the Zeiss LSM 700 scanning confocal module shall only be used in accordance with the manufacturer's instruction manuals. 2. Users who experience any malfunction with the hardware or software of either system shall report the problem immediately to the RIGSC manager. 3. Users of this equipment are not permitted to troubleshoot, diagnose or repair any malfunction of this equipment. 4. Users of this equipment are not permitted to remove any covers of the laser light sources and mirror assembly or change the path of the laser light beam in any way. 5. DO NOT look directly into the LASER beam. 6. DO NOT look at the radiation reflected from the specimen slide at a distance closer than 20 cm. Wear eye protection at all times when using chemicals, solvents, and vacuum equipment. 3. In case of accidental contact by a chemical with your skin or eyes, rinse with plenty of water using the hose at the sink. Notify the Manager, as soon as possible. 5. Wear disposable nitrile gloves when using any hazardous chemical. 6. Never pipette any chemical or solution by mouth. 7. Read and understand the Material Safety Data Sheet for all chemicals that you are working with. 8. Dispose of used microscope slides in the sharps container 9. Dispose of all hazardous wastes in the appropriate container. 10. Do not dispose of hazardous waste in sink drains. 11. Report any chemical spills to the Manager. In case of emergency or spill of a toxic chemical, LEAVE THE AREA, first. 12. Wash hands immediately upon leaving the laboratory. 13.
