 Personal Statement describing post-college plans and goals, as they
pertain to your field of study, in 350 words or less.
 One letter of reference from a teacher.
 Resume with academic achievements, extracurricular activities, community
service and work experience.
 High school students: provide a certified high school transcript for grades 9-12.
 College students: provide a certified transcript from the college you are currently
attending. The transcripts must show current enrollment of at least 12 units.
