Residential Manager / Team Leader (s) Lowther Street (24 hour) Crisis House Whitehaven (This entails 15 hours Team Leader role and 15 hours Residential Project Worker) Job Purpose To provide short term support to service users who are experiencing a mental health crisis and find themselves in need of short term 24 hour staffed supported accommodation Dimensions Number of staff managed: Budget Responsibility: Liaison with external agencies: 0 Petty cash only Crisis Resolution Home Treatment Teams (CRHTT) Community Mental Health Teams (CMHT), Early Intervention Psychosis (EIP), Assertive Outreach Teams (AOT), Acute Inpatient Services, G.Ps, Housing agencies Organisational Chart Residential Manager Team Leader (s) Residential Project Worker Key Duties To act as a key worker for a number of residents within the project, carrying out needs assessments and formulating support plans, risk management plans To provide appropriate short term recovery based support. To undertake therapeutic interventions aimed at resolving the current crisis and communicate hope. To assess the needs and risks as relevant to each Service User and to support them to set and achieve their identified outcomes. To monitor and record the Service User’s progress by a process of regular review. To signpost people to a range of services and organisations, facilitating liaison between service users and other agencies. Accurately maintain all service user recording systems paper based and computerised to enable monitoring & reviewing To Participate in service user reviews and provide monitoring information as required. To promote collective responsibility and active participation in the Project milieu To facilitate resident meetings, therapeutic groups/activities. To Carry out daily household chores To work within policies and procedures; in particular Health and Safety, Lone Working, Confidentiality, Professional Boundaries, Disciplinary Rules and Health and Care Professional Council ‘Standards of conduct, performance and ethics.
