Liberty Christian Academy Preschool Parent Agreement
We/I, __________________________________________________, the parent(s) or legal
guardian(s) of _________________________________________________ hereby agree:
1.
For preschool/daycare given to the above named child, the parent will pay monthly
payments which are due on the 1st of each month, with the first payment being due on the 2nd of
September. Payments made after the fifth day of school at the beginning of the month, will incur a
10% late fee. A non refundable registration fee of $50.00 and a non refundable book fee are due
upon registration. Half day students remaining at the center after class times: $8.00 an hour.
Hourly fees are to be paid when the child is picked up. Late Payments: Any fee and/or tuition that
is not paid at the end of two weeks after such fee and/or tuition were originally owed; the child
will automatically be dismissed unless the parents make definite arrangements for payments.

2.
The parent will not violate the hours of care set forth above in paragraph one (1).
3.
No refunds will be issued for missed days, and there will be no make up days.
4.
In case of illness, or accident: When a parent cannot be contacted, and in the judgment of the worker or
staff, if the illness, or accident requires a physician, the parent hereby authorizes that

5.
In all emergencies, parent hereby grants permission to take such reasonable measures as are, in the
judgment of the worker or staff, necessary to the welfare and safety of the child.

6.
In the event of contagious illness, the parent will notify the school and remove the child
and not allow him/her to return until all danger of contagion is past. LCA reserves the right to request a note
from your doctor stating that the child no longer poses a risk to others.
7.
Parent has filed with the Academy a current health report, immunization records,
custody statement, and birth certificate for their child. Parent has also given full details and phone
numbers where they can be reached at all times that their child is in our facility or care, as well as
an emergency contact number. Parent agrees that the following symptoms are cause for keeping
their child home; cold, coughing, flushed skin, earache, red eyes, chills, headache, sneezing,
rashes or other skin eruptions, sore throat, fever, and vomiting.
8.
LCA reserves the right, and has sole discretion, to dismiss any child if after entering he/she is unable to
participate in the group experience.
9.
Liability for the acts of child while he/she in under the care of LCA is the parent's responsibility.
10. LCA is not responsible for accidents or illnesses occurring to the child while in its care,
unless it can be proved that the accident was the direct result of negligence. Parent submits to the jurisdiction of
the Mesa County Court and the laws of the State of Colorado.
11.
Parent will give two (2) weeks notice when withdrawing the child from the care of LCA.
BOTH PARTIES - Liberty Christian Academy and Parent understand and agree that:
A.
This agreement is a mutually binding contract.
B.
This contract may be terminated by either party upon notice as set forth heretofore, or at
any time by mutual agreement of the parties.
