Infectious Diseases/Antimicrobial Stewardship Clinical Team Leader – Carolinas Medical Center, Charlotte, NC Description: Carolinas Medical Center (CMC), an 874-bed teaching hospital located in Charlotte, NC, has an immediate opening in the Department of Pharmacy for a Clinical Team Leader in Infectious Diseases. CMC is a national leader in specialized care for a broad range of medical conditions. The individual will play an integral role in the antimicrobial stewardship program and pharmacotherapy management of infectious diseases. Additionally, the clinical team leader will provide clinical and operational support to the Department of Pharmacy as well as serving as a key member of the interdisciplinary health care team, providing clinical support and teaching to physicians and nurses. The Clinical Team Leader will be responsible for education and mentorship to clinical staff pharmacists, PGY1 residents, and pharmacy students. Other responsibilities include participation and leadership in pharmacy-related quality improvement initiatives as well as policy and/or guideline development and formulary management. Key Responsibilities: Daily monitoring of targeted antimicrobials and appropriate utilization. Provide daily feedback and recommendations to practitioners for optimal use of antimicrobials. Documentation of interventions and daily review with CMC ASN physician. Rounding with Infectious Disease Teaching Consult Team Work with current pharmacy pathways to ensure the appropriate pharmacokinetic and pharmacodynamics utilization/dosing of antimicrobials. Perform surveillance of current PK/PD dosing pathways and provide pharmacy feedback on improving utilization of these protocols. Liaison with Microbiology, including participation in the antimicrobial testing panel selection process and production of antibiogram. Work in coordination with CHS Antimicrobial Support Network and Infectious Disease Coordinating Committee to determine new agents and their place on formulary, changes in recommendations or dosage of current antimicrobials on the formulary, or disease based antimicrobial guidelines. Collaborate with CHS Antimicrobial Support Network in obtaining and analyzing quantitative and qualitative data on antimicrobial drug use as it relates to patient care, safety, drug utilization, and economic outcomes including prescribing patterns, drug utilization, and disease-state reviews in selected patients. Coordination and development of clinical conferences newsletters and educational forearms for health care professionals focusing on ongoing antimicrobial stewardship programs, appropriate antimicrobial use, and clinical guidelines. Provide education materials, in-services, and lectures about antimicrobial stewardship to CMC clinicians including nurses, mid-level practitioners, and physicians. Precept and mentor pharmacy practice and infectious disease specialty residents. Participate in research projects. Qualifications: Doctor of Pharmacy Degree (PharmD) from an accredited college of Pharmacy, clinical pharmacy residency or equivalent experience and licensure by NC Board of Pharmacy required. Two years as a clinical pharmacist with involvement in formulary development, medication use evaluation, and quality assurance activities required. Successful completion of a PGY2 Infectious Disease Residency is preferred. Applicants with equivalent experience in infectious diseases and antimicrobial stewardship will be considered.
