BHS PORTFOLIO GUIDELINES AND CHECKLIST
REQUIREMENTS:



Student’s name and Culminating Project topic must be neatly stated on outside of portfolio.
Portfolio should include an inside cover page stating: student name, culminating project topic,
essential question and name of community advisor.
Dividers between sections need to be neatly labeled and visible. The following section dividers
are recommended: Proposal, Scholarly Paper, Time Log and Community Advisor (final
observation form should go in this section), Reflection and Artifacts.
THESE DOCUMENTS MUST BE INCLUDED IN APPROPRIATE SECTIONS:

Culminating Project Approved Proposal and Rubric A (Completed and Signed) – These
documents permit the panel of judges to note each student’s initial overview of all of the
components of the project.

Approved Scholarly Paper and Rubric B (Completed and Signed) – This academic writing
allows the panel of judges to understand the scope and depth of the student’s research into a topic
that may or may not relate to the project’s fieldwork. In addition, Rubric B permits the panel
members to note the student’s ability to research and draft an academic essay utilizing MLA
format and citation.

Signed Statement of Integrity (This may be attached to your proposal).

Completed Time Log (Page One Signed by Parent/Guardian & Page Two signed by
Community Advisor) – These account for the hours spent planning and participating in fieldwork
as well as for time spent collaborating or reflecting with the Community Advisor. Page two also
requires the student’s signature to verify completion of the hours logged. Note that time for
putting together portfolios and writing proposals, scholarly papers and written reflections does
not go on the time log!

Community Advisor Final Observation Form (Completed and Signed) – This document
provides the student and the panel of judges with feedback regarding the student’s performance.
The Community Advisor’s signature verifies satisfactory completion of the student’s fieldwork.

Written Reflection – This two to three page essay elaborates upon the content required by Rubric
C: a description of the project, an explanation of personal growth and new learning, a description
of challenges, and an explanation of the project’s connection or benefit to the community.

Artifacts – These items, along with the previous documents, offer proof to the panel of judges that
the project did, indeed, connect to or benefit the community. In addition, these objects
demonstrate the progression of the fieldwork of the project.
SENIOR PORTFOLIOS MUST MEET STANDARD BY THE DESIGNATED DUE DATE IN
ORDER FOR SENIORS TO BE ELIGIBLE TO DELIVER PRESENTATIONS.
