TEACHING PRACTICE
We are a teaching practice supervised by the London Deanery. Our current
GP Registrar is Dr Tasneem Kapasi who will be based at the practice during
the period 05.08.2015 – 31.03.2016.
MEDICAL STUDENTS
Our practice is a community teaching practice for Kings College London
School of Medicine. With the supervision of your doctor or nurse, medical
students may, with your consent in advance, to join your consultation. This
is one way medical students learn to become good doctors and understand
patient views about their care. We appreciate patient’s help with their
learning. Please let your doctor or practice manager know if you have any
questions about our community medical teaching.

The practice offers a wide range of clinical health care services. These

Health checks to all new patients and annual health checks for existing
patients
Health Checks for Dementia/Cardio/Learning
Disabilities/Depression/Asthma/COPD
Some of these services are offered on a defined clinic basis others as part of
the routine care.
INTERPRETERS
The practice has access to a full range of interpreter services. Please ask a
member of the reception team for further details.
SURGERY HOURS
Monday-Friday 7.30am – 6.30pm
(Please note that the Practice Nurse has early morning clinics Mon-Thurs
with pre-booked appointments only).
APPOINTMENTS
Appointments are available up to six weeks ahead but please note they may
not always be available with the clinician of your choice. Whilst every effort
is made to accommodate patients this is not always possible.
EMERGENCY APPOINTMENTS
There are a limited number of same day (emergency) appointments Mon- Fri
available with the Duty Doctor.

Same day appointments are also available at the local Urgent Care Centres in
Wembley, Cricklewood and Central Middlesex Hospital. Please speak to a
member of the Reception Team who will advise you.
WILLESDEN HUB CENTRE
Same day appointments are available at the Willesden Hub Centre. Please
speak to one of our Reception Team who will book you an appointment using
our template forms.
URGENT CARE CENTRES
Walk in appointments are available at the UCC at Central Middlesex Hospital,
Wembley Centre for Health and Care and Cricklewood Lane.
OUT OF HOURS EMERGENCY COVER
When the surgery is closed advice may be obtained by telephoning BARNDOC
on 03000 333 777.
IF IT’S NOT A 999 EMERGENCY – CALL 111
This is a new NHS telephone number that will make it easier for you to access
local health services. Patients are able to call 111 when you need medical
help fast but it is not a 999 emergency. When you call you will be assessed,
given health advice and directed straight away to the local health services
that will help you best. This could be A&E, an Urgent Care Centre (UCC), an
Out of Hours GP, Community Nurse, Emergency Dentist of late opening
Pharmacist.
Patients are able to call 111 – 24 hours a day, 7 days a week, 365 days a year.
Calls from landlines and mobile telephones are free.
HOME VISITS
Home visits to patients who are genuinely too ill to attend the surgery may
be made. In order that a doctor can attend as soon as possible please
telephone the surgery before 10.30am. Additional clinical help is available
at the surgery so please endeavour to attend the surgery if at all possible.

REPEAT PRESCRIPTIONS
To order these please fill in the Repeat Prescription request form on
reception and place in the Repeat Prescription Box. Please allow 48 hours (2
working days) for prescriptions to be prepared. Prescriptions are issued at
the prescribing doctor’s discretion and the doctor may decide you need to be
seen here before a prescription may be issued. Alternatively you may fax
through your request to 0208 208 8409 or email through our website.
NORTH WEST LONDON DIABETIC EYE SCREENING PROGRAMME
There is a new programme website www.nwldesp.co.uk for patients. This
will help keep patients informed about diabetic retinopathy, their
appointment and booking office number to change the date, time or location
of their appointment. Booking Office Number : 020 8099 1122.
ADDITIONAL PAPERWORK REQUESTS FROM PATIENTS
Any additional forms/letter request should be passed to the Practice
Manager and NOT to the doctors as this may delay the process. Please allow
5 working days for these to be processed. Also note that some non-NHS
forms/letters may be chargeable. These may include assessments for
employment, insurance or social services agencies.
NON-NHS SERVICES
Some services are not covered by the NHS. Please make an appointment to
see the Practice Manager for clarification.
CHANGE OF PERSONAL DETAILS
Please inform our Registration Manager Mrs Barbara McAndrew of changes
in name, addresses and telephone numbers in order that we are able to
contact you as soon as possible if need be.

TEST RESULTS
Test results are available from the Practice Nurse. Please leave a message
with a member of the Reception Team and they will pass this to the Practice
Nurse who will call you back with your results. Please do not ask the
Reception Team to give you/explain your results as they are not qualified to
do this.
CHAPERONE SERVICE
All patients are entitled to have a chaperone present for any consultation,
examination or procedure if they wish. This chaperone may be a family or
friend. On occasions you may prefer a formal chaperone to be present ie a
trained member of staff.
DISABLED ACCESS
There is disabled access in the form of a ramp at the side of the building
which leads to rear access into the practice. There are also two disabled
parking spaces at the front of the building.
