Family Preservation Supervisor-Networks
RE: Services provided by DSS/ How schools access services

The family preservation workers are available for the following:
 Providing case management services for families that have been identified as vulnerable
or in needed of family preservation support (Schools should forward request to the DSS
supervisor).
 Participation in SST meetings to help identify needs and provide support to families via
referrals or case management services.
 Participation in attendance hearings/meetings to help identify and connect families with
services needed.
When sending referrals for attendance (truancy- chronic absenteeism), behavior/conduct
problems, social/emotional concerns, and McKinney-Vento Eligible students (MVEhomeless), please do the following:
1)
Ensure that all school protocols are followed (calls, home visits, social worker, etc).
2)
Ensure that McKinney-Vento students are registered in the school system as
homeless and connected to needed services.
3)
Identify stressors that impact family functioning (See below).
4)
Contact the family preservation worker to discuss case (if consultation is needed) –
visit http://www.baltimorecityschools.org/Page/484 for the DSS supports on each Network.
5)
Complete and forward the referral to the DSS supervisor Tonya Socfield.
To make a referral for services on behalf of a student/family complete the attached Family
Preservation referral and forward to the DSS supervisor Tonya Scofield.
Referrals should be based on identified issues that impact family functioning, wellbeing, and permanence. This includes:

Substance Abuse in the Family
