These infections occur mainly as a result of contamination from an unclean environment and from harmful delivery practices, including the use of unclean materials during delivery. The high rates of home deliveries by people with little or no training on hygienic delivery practices, shortages of clean (recommended) delivery materials all contribute to the problem of perinatal infection. Clean birth practices combined with monitoring and active third stage management may prevent up to 23% of maternal deaths in low income countries including Nigeria... Hassan et al. found that unhygienic practices are common among traditional birth attendants (TBAs), but also evident among skilled birth attendants who work within health facilities. Every single day, Nigeria loses about 2,300 under-five year olds and 145 women of childbearing age on pregnancy/delivery complications including infection which accounts for 15 percent of this burden. This makes the country the second largest contributor to the under–five and maternal mortality rate in the world. Eighty percent of pregnant women in rural communities attend ante-natal classes and less than Ten percent deliver at the health facility. They prefer the traditional homes due to low cost of deliveries which has very poor, dirty and unsafe environment for delivery. WHY THE CLEAN BIRTH KITS? Each year, thousands of women living in rural communities globally including Nigeria give birth on their own or without the help of a trained health worker. These births often take place at home, where the risk of infection is high. At TFCH every Mother and child in the world is so precious to us, we are committed to saving lives and we are crossing borders to achieve this e.g. (Niger Republic). Also, many women are turned away from hospitals and clinics if they do not bring their own delivery materials with them. At home or in the hospital, inadequate basic materials provides an opening for infections and other complications. The clean delivery kits are often viewed as luxury, which the rich alone can afford to aid safe delivery. While women in rural communities have little or no knowledge of what the kit looks like, let alone being able to afford it. Some healthcare providers in rural Nigeria are found to have delivery room that are dusty and dirty, tools are rusty and blood laden from the last birth; and there is no way to sanitize the instruments. As a result, unsanitary conditions persist leading to increased rates of infection. Clean birth kits are globally recognized as an effective solution. According to the World Health Organization, when birth attendants are educated about birth kits and how to use them, it has a significant effect on reducing maternal and infant mortality rates. To respond to the above challenges, the clean delivery kit is a simple approach to reduce this risk. These kits help to provide a clean birthing environment for mothers and newborns who would otherwise be left to go through the birthing process in an unsanitary environment that creates risk of maternal infection at point of delivery .Provision of free clean birth kits to our maternity homes / Primary health facilities provides a safer birth environment and better health outcomes for mothers and babies. In respect to this, TFCH is currently using the clean birth kits as a tool to reduce maternal and infant mortality within rural communities in Nigeria, thus generate strong support for women in rural communities. The birth kit is created by women, packaged by women and used to save women from unsanitary birthing condition, thus saving the WORLD There’s no greater gift for a mother than a healthy newborn and being alive after delivery. But not every mother is fortunate to give birth in a clean hospital delivery room. With this innovation, Traffina Foundation will provide women in resourcepoor settings (direct beneficiaries) with increased access to a clean and safe delivery and a healthy start to a new life through provision of free delivery kit for rural women. The kit is given to the mother as a gift, recognizing childbirth as an important and courageous act of womanhood – a feeling that many mothers never get a chance to experience in many cultures. TFCH has a target of providing 50,000 free clean birth kits to improve access to maternal, new born and child health in Nigeria by end of 2015. Currently TFCH has provided six thousand (6000) free birth kits across the following the states in Nigeria, thus saving more than 12,000 lives since 2013: Nasarawa, Kebbi , Niger, FCT Abuja , Imo and Abia states in Nigeria This approach has created a lasted positive behavioral change in mothers on how to care for their new born, improved access to quality care at health facility due to relationship the clean birth kit has created between our midwives and the mothers. This has also helped in HIV/AIDS prevention treatment and care for the new born that would otherwise die without having access to health care. The clean Birth kit contains the following 1. ONE LONG LASTING MOSQUITO TREATED NET: 10. ONE BABY DIAPER: 11. 2 PAIRS OF STERILE GLOVES 12. Immunization calendar / reminder cards in local languages. 13. Thermometer. 14. Mucus extractor. 15. Long lasting mosquito treated net. The overall objective of this modest initiative is to contribute to the reduction of maternal and perinatal morbidity and mortality through clean and safe delivery. Project Objectives: To provide 50,000 clean birth kits to support pregnant women in poor resource. • To provide information to women, men in communities on the importance of delivering under a clean environment. • To provide free clean delivery Kits to women in rural communities who cannot afford to pay for the cost of delivery even at primary health center. • To build capacity among community leaders, to create awareness on clean and safe delivery kits. • To establish a monitoring and evaluation system for implementing the “1 KIT SAVES 2 project. Expected outcomes (1) improved reproductive health service delivery; (2) Saving more than 100,000 lives (women and children) from dying due to delivery infection by end of 2015 (3) Improved utilization of the clean birth kit and related services by expectant mothers (4) Increased rate of facility delivery / Improved service utilization. (5) Increased community awareness on importance of safe and clean delivery outreach services (6) Reduced traditional home births (7) Reduced record of infection at delivery ACTIVITIES This project aims to increase access to improved knowledge of life-saving tools, which will provide clean and infection-free delivery to less privileged mothers: 1. Community advocacy and sensitization to community heads. 2. Radio/TV jingles in local languages. 3. Community sensitization for all women, Traditional birth attendants and health facility staff in the communities to create awareness on the free clean birth kits. 4. Distribution of kits to facilities and 1day orientation for health facility staff on the use and storage of the kits. 5. Collection of monthly reports on number of women who utilizes the kits including their contact for verification. 6. Monthly supportive supervision on the project. 7. Quarterly supportive supervision to monitor the effectiveness and challenges on the project. 8. Training of Midwives and community volunteers.
