Human test subjects are to sign on this form each time you meet for a test session / interview session.
This form must be submitted along with the Reimbursement / Invoice Payment Form for all reimbursement claims
for the payment of human test subjects. You will have to pay the subjects first. Office of Finance will reimburse the
amount claimed directly to the account indicated for your salary payment.
