complete and return in order to participate in our end of year activities. Please carefully read
over the following forms. Information on Prom, Post Prom, Senior trip, and Graduation has been

 Ceremony letter with details on dress code, parking, and behaviors
 Graduation contract/media release form must be signed and returned

Senior Prom and Post-Prom
 Details on event locations, dress code, rules of behavior and attendance
 Prom permission slip must be signed by a parent and guest’s parent and returned
 Post prom permission slip must be signed by a parent and guest’s parent and returned

 Allows students to leave early on the day of the Prom (6/5)
 Permission slip must be signed by a parent and returned by FRIDAY, MAY 29th
