to Pastor Dave at revcady@gmail.com. If you are using a hard copy of this document, please type your
responses on a separate sheet, attach to this document, place in an envelope, and give it to Chris Mills or Penny
Sullivan in the church office. Applications are due by June 23, 2013 and will only be seen by the Lead Pastor
and District Superintendent.

confidential; that they be shared with no one outside the board. Are you able to commit to absolute
confidentiality in this leadership role?

At least one time per year the Breakthrough Leadership Board will, attend a training event. Are you
able to commit to participation in an annual training event and accept responsibility for your costs, if
