Delaware Music Boosters Music Leadership and Commitment Scholarship – Student must have
been active in a music program all four years of high school, and the scholarship will be awarded to
a student who has shown a high level of commitment and leadership to the music program. Attach a
completed activity sheet and answer the following question: In what ways has your leadership and
involvement in the music program prepared you for your future?
Completed applications (including resumes, letters of recommendation & additional responses) are due
April 1st (no exceptions!) to the Counseling Office. This application should be completed without the assistance of
family members, teachers, peers, etc. See your School Counselor with any questions.

Community Activities (Please include the name of each community activity you participated in without pay
during the last 4 years, number of years participated, and any special honors or award.
