NHS Health Checks
Age group is 40 – 74 yeard olds with no DDM’s (Chronic
Disease Monitoring). Paula informed the group that from
January the amount we will receive as a practice will be greatly
reduced although the final figure has not been decided yet. Fi
mentioned risk scores for health checks.
Paula went onto explain to the group how it works.
Patients are seen for Part 1 which covers everything ie BP,
Bloods, Weight, Height etc then 2 weeks later the patient
returns for the results. Dr Helen McClelland sees the results
and decides whether the patient is high risk, medium risk, low
risk or no risk.
High risk – needs to see a Doctor urgently
Medium risk – Needs to Doctor
Low Risk – see Nurse or Health Care Assistant
No risk – telephone call
Every patient gets a result and a report.
In January when the cost reduces the amount of work will
reduce as the only thing that will be tested for is Cholesterol.
Paula mentioned to the group that at her Practice Managers
meeting they decided as a group the the funds on offer were
not sufficient to cover the work being done. The federation
North East and West will cost out. The Practice Managers
suggested a figure of £30 per patient and the answer was
declined by the Local Authority and negotiations ended. Fi
suggested Paula write to the Chief Medical Officer for England
and Wales.
Paul wanted to know who was entitled to an annual check. He
mentioned the Open Door policy at the hospital to look at
medical records and asked what about the surgery. Paula said
that here we have restricted access to our patient medical
records.
Diabetic Patients
The Practice does invite Diabetic patients in to the practice for
a health review if their diabetes is being monitored by the
Hospital, however many patients do not want to be seen twice.
Friends and Family
Comes in in December. Will be advertising on the website and
surgery.
There will be certain questions that are mandatory. The
Practice is designing their own forms. There will be monthly
updates and we will be audited. Paula asked the group for
help with implementation if thought appropriate
Final score will be for what we have done. Steve asked if we
had seen a table and then produced a hospital copy. Everyone
seemed to think this looked confusing.
Apprentice Skill Academy
The group were informed that Charlotte was leaving on the 20th
October has she had obtained a full time job.
Abby was our new Apprentice
Kathryn finishes her annual apprenticeship she is going to
become a Health Care Assistant in November which is the 1st
time this has happened.
Items Raised by the Group
Communication beyond the group:
Jean and Paula met during the Summer to discuss
communication beyond the PPG, with other patients and other
PPG’s for example. Jean had prepared a discussion paper.
The outcomes:
A newsletter would be produced for distribution.
A noticeboard would be in place for items of PPG interest.
The Acorn Group would be used as the contact with other
PPG’s.
Coffee morning style mingling was thought inappropriate.
Volunteers would be sought via email.
Jean informed the group that NAPP had been set up with help
from a couple of members of the group and that a gmail
account was in place.
Fi asked how to use sytem1 appointments/medication online
once established. Once she had obtained her username and
password it was pretty straight forward.
Encouraging patients to keep themselves healthy:
It was decided that the group would collectively gather
information on healthy activities locally to promote good health
amongst patients. A display/file would be created. Again help
was requested with information to be sent to Jean.
Fi asked about finding out the correct ways of delivering
information to the group – CQC. Fi asked if it would be
possible to get someone from the CQC to come and talk to the
group and Paula offered to contact someone.
GP Practice Boundaries – Postponed until January meeting
Implications named GP – Postponed until January meeting
Comunication & responsibility of Patients – postponed till Jan.
Feedback from Acorn Group
Paula informed Anita that the Practice was happy to
communicate through the Acorn Group

in April by the NHS but was unsure whether there was going to
be an alternative.
