AS SO CI A T E HEA LT H CA RE A NA L YTIC S Social Finance About Us Social Finance is a not for profit organisation working in partnership with government, the social sector and the financial community to enable sustainable social impact at scale. We are the leading social investment intermediary and a centre of innovation in addressing social issues in the UK and beyond. With teams working with Central Government departments, Local Authorities and Clinical Commissioning Groups, Global Development Organisations, Charities, Social Enterprises, Foundations and Investors, and with experts in their fields, Social Finance brings to bear a deep understanding of a range of complex social issues to the proposals it develops - in areas like financial inclusion, affordable housing, recidivism, homelessness, domestic abuse, improving care for those with long-term health conditions and children in or at risk of entering care. Its work in the development arena has focused on neglected tropical diseases, HIV, education and employment. Since we started in 2007, Social Finance has mobilised over £52 million of social investment and designed a series of programmes to tackle social challenges. These include support for 2,000 short sentence offenders released from Peterborough Prison, 380 children on the edge of care in Essex, 2,500 young people at risk of becoming NEET, 600 homeless youth and 800 rough sleepers in London. We are also alleviating fuel poverty for over 2,300 families in Sunderland, enabling 15,000 families to access nursery places and free children’s services, and providing 7,500 affordable micro loans in Wales. About the Team The Health and Care practice looks to develop and scale more effective approaches to improving people’s health and wellbeing, particularly focusing on community based and preventative approaches. We have developed significantly recently, and are looking to grow our impact and scale further. Over the last year we have: Launched the Shared Lives Incubator to invest in rapidly growing Shared Lives schemes – care provided to people from within the carer’s home. Established the Reconnections Service to support 5,000 older people in Worcestershire to overcome loneliness and isolation, to improve wellbeing and reduce use of health and care services and replicate the model nationally. 2 Developed a major new social investment fund in partnership with a major health charity - which will be launched in October to invest in better services for those affected by cancer and other long-term health conditions. Pioneered investment in services which better integrate health and employment support. Helped a number of social enterprises to develop business plans for improving scale and sustainability, such as Salford Health Matters (one of the UK’s leading primary care practices), and commissioners to redesign their services, such as Newcastle City Council’s learning disability service. Our partners include Nesta, the Health Foundation, the Cabinet Office and Department of Health, Big Society Capital, the Big Lottery Fund, Imperial Health Partners, the Gulbenkian Foundation, the Esmee Fairbairn Foundation, the John Ellerman Foundation and Community Catalysts. Our Goals and Values We are focused on improving the impact of health and care services on people’s health and overall wellbeing. We look for social impact first – our models are driven by social and then financial returns – and are looking to develop approaches which both practically benefit the immediate users of the service and have the potential to scale and be replicated nationally.
