Job description Job title: Bone Health Exercise Instructor Grade: E Hours: Full time Responsible to: Falls Coordinator Accountable to: Director of Public Health Location: Kingston Public Health Team Contract: 1 year 1. Job Summary: Provide osteoporosis screenings to people 50+ years old in Kingston in community settings. Provide group exercise programmes for people with bone health conditions including osteoporosis and osteoarthritis. Hold education sessions to service users on bone health, exercise and nutrition. Work in collaboration with the Bone Health and Exercise Specialist and provide administrational support with referrals and data collection. Provide bone health risk assessments over the phone and in person when needed using the FRAX tool. Support people living in sheltered housing accommodations by providing advice on how to achieve optimum bone health including nutrition and exercising and general lifestyle advice. Represent the Bone Health Service at community events. Keep Service database up-to-date. Key Working Relationships The post holder will be required to liaise with a broad range of health care professionals, third party organisations and internal and external stakeholders; including the Kingston Falls Service, GPs, Kingston Hospital, sheltered housing schemes and care homes. Support the work of the Falls Coordinator and Bone Health Specialist to develop and implement project data collection systems that will provide accurate and timely data. Report regularly on issues, referral numbers and service uptake to falls coordinator. Support the work of the Bone Health and exercise specialist. 1 Work in partnership with the Kingston Falls Prevention Service and Kingston Hospital Falls Navigator to ensure the local bone health pathway is integrated with the local falls prevention pathways. Refer patients onto other exercise interventions when appropriate. 3. Functional Responsibilities Maintain data collection systems to ensure the progress and outcomes of the project can be monitored adequately. Provide information to falls coordinator on the development of the project and statistical information matters. 3.1 Service Provision Provide evidence based osteoporosis risk assessments and basic falls assessments face to face and over the phone when appropriate. Support the mapping and securing of appropriate venues for group exercise programmes. Follow our standard programme and provide evidence-based group exercises for people over the age of 50 years who suffer from a bone health condition. Organise and hold educational sessions to the public on bone health, nutrition and exercising. 3.2 Research and Development Actively support and contribute to the development of key performance indicators for successful assessment of performance. 4. Operational Responsibilities Support the implementation of the Service Specification through timely and relevant information analysis and administrative support, in accordance with agreed priorities of the team. Deliver against agreed objectives, achieving quality outcomes. Plan, organise and manage bone health screening sessions face to face and over the phone and group exercise sessions. 5. Policy and Service Development Make recommendations for more effective service delivery. Contribute to the review and development of the Kingston Bone Health pathway and to the development of an integrated approach to falls and fracture prevention in Kingston. 2 Person Specification Supporting Evidence In the supporting evidence of your application form, you must demonstrate your experiences by giving specific examples for the criteria within the person specification. Factors Knowledge, training and experience Description Educated to degree level in Sport science, Physiotherapy, Physical education or in a similar subject area. REP or equivalent registered Experience of working at a similar level in falls prevention or bone health or exercise provision.
