The student must submit a completed form (Sections 1-7) to their Faculty

Advisor(s). The Advisor(s) will fill out Section 8 and submit the final copy to OVC Graduate Program Services via email, cc. the student.

Important: Incomplete applications may be rejected. The recipients of the awards (except those with financial need as the sole criteria) will be announced throughout the university community and posted on the OVC website.
