SECTION 10: ADMISSION/DISCHARGE/TRANSFER POLICY #1: ADMISSION Effective Date: 4/93 Revisions: 2/14, 2/00, 2/07 POLICY: Criteria for admission into any residence operated by New England Residential Services are as follows: primary diagnosis of mental retardation, availability of funding, and acceptance into a day program. No applicant will be discriminated against because of race, creed, religion, citizenship, national origin, age, sex or sexual orientation. All efforts will be made to meet the needs of the prospective applicant and the individuals already living at New England Residential Services. The admission process begins with a completion of a “Notice of Opportunity” form. This form is submitted to the appropriate DDS region. When DDS makes a referral the process continues with research into the applicant by the placement team and subsequent selection. The transition/pre-admission process begins with conversations and meetings with anyone on the sending team who might have information and input. Some examples would be family, axillary helpers to the family, day services staff, therapists, doctors, etc. During this time the residential director will schedule visits to the residence for dinner, overnight and the weekend. If a prospective resident has a family member or respite worker assisting during the transition period, those support people will have sole supervisory responsibility during this time. A transition meeting will be held to ensure that all actions needed for a smooth transition are completed and to set a date for move into the house. An Individual Plan meeting will be scheduled within 45 days of the move to finalize formal admission and to plan for the coming year. SECTION 10: ADMISSION/DISCHARGE/TRANSFER POLICY #2: DISCHARGE Effective Date: 2/14, 4/93 Revisions: POLICY: A person may be discharged from New England Residential Services upon the decision of the interdisciplinary team for the following reasons: 1. The individual has indicated a desire to move away from the agency. 2. The individual has demonstrated a need for an environment which offers supports that the agency cannot provide. Once the team has made/approved this decision, DDS will be contacted to locate a new residence. New England Residential Services staff will assist with planning, transition and consultation after the move if needed. All internal transfers must be approved by DDS. When an individual moves to another agency, the residential manager will responsible for completing a discharge information inventory which consist of records and financial information.
