Request that your scores for the Graduate Record Examination (GRE) be sent to:

STEP 2: Official Transcripts
Request that official transcripts (one set) from your previous college(s)/university(ies) be mailed to:

Request your transcripts early so they are sent out to be postmarked by February 1.
FGCU transcripts do not have to be requested.
STEP 3: Application Packet

 A cover letter, in business format, which states your intent to apply to the program
 Your personal statement, as per enclosed guidelines
 Documentation of Volunteer/Work Experiences (2), as per enclosed guidelines
 Completed Department of Occupational Therapy and Community Health Supplemental

Adobe format)
