(This petition should be used by students not enrolled at Ohio University four or more years.)

This Request for Readmission should be completed if you are applying to return to Ohio University after an absence of four or more years
and must be submitted by August 1 for fall semester consideration or by December 15 for spring semester consideration.
•
Requests received late will be held for review the following academic term.
•
Submitting the Request for Readmission is not a guarantee of reinstatement to the university.
•
The Request for Readmission should be legible, reflect an honest assessment of your strengths and challenges, and be completed with
care and deliberation.
•
If you are off campus, you may mail your Request for Readmission to University College, 140 Chubb Hall, Athens, Ohio 45701; fax to
740-593-0206; or e-mail to university.college@ohio.edu. If you are a regional campus students, please submit your Request for
Readmission to a student services advisor on your campus.

PLEASE NOTE: This Request for Readmission to Ohio University addresses only the possibility of reinstatement to the university and not
the status of your financial aid. It is possible that your financial aid may be suspended even if you are reinstated to the university. You are
strongly advised to contact the Office of Student Financial Aid to determine your eligibility for financial aid.

1. What have you been doing since you were dismissed? What motivated you to seek re-entry at this time?
2. What is your proposed major? Indicate the accumulative grade average required for admission and list any
course requirements you must complete to be admitted to that major? What leads you to conclude that you will
be able to meet the requirements to gain admission to your major of choice?
3. List two goals you expect to achieve within the next ten years.
4. Indicate reasons for your previous unsatisfactory grades. What circumstances have changed that lead you to
believe you can improve your past academic performance? Describe the steps/plan of action you will follow to
ensure that your grades improve if you are allowed to return.
5. List courses you will take next semester and state why you chose each course. Indicate which of your planned
courses are retakes.
You might be interested in the Application for Segmented Transcript if you have been absent from the university for a
period of four years or longer. DO NOT return the Application for Segmented Transcript until you are enrolled and
attending classes.
