PARENT-CHILD DAY
SHADOWING is an opportunity for you to spend a designated amount of time, usually two hours, with a parent or
person who serves as a career model in the area of your career interest. You fill out a registration form, bring in
your parents’ signature and signature of the career contact person. Please read with your parents the attached sheets
and sign the release form if you want to participate in the program.
RELEASE FORM
To Students and Parents or Guardians:
The staff of the Parent-Child Shadowing Program and all Community Sponsors will make every effort possible to
assure the health, safety, and welfare of all participants in the Shadowing Program. Despite all efforts and
precautions, it must be anticipated that an emergency, illness, or injury may affect students participating in the
program.
Neither the Community Sponsors, the Shadowing Program, any employee or appointee thereof,
Schools, nor any party, organization or agency collaborating with the Shadowing Program
is or shall be liable for any injury, loss, damage, deviation, delay, or curtailment, however caused, or the
consequences thereof, which may occur during any portion of this program.
I have read the above paragraphs and do accept the statements set forth.
