Junior Community Service Completion Slip
This slip is proof to your Advisor that you have completed the required 1 HOUR of
community service off campus. Please return this slip to your advisor once completed.
Remember that this hour of community service is a requirement of Junior Advisory, and
you will fail advisory if you don’t complete it by the due date. This service may be a part
of something that is an ongoing commitment in which you participate.

Contact: It is a requirement for all juniors to complete an hour of community service
somewhere outside the high school in order to pass Advisory. We ask that students
get the signature of an adult who observed, organized, or advised their act of service.
