Library Book Permission Slip - Rochester City School District

advertisement
Dear Nathaniel Rochester Community School Family,
I’m looking forward to meeting all your children in the library. All grade K-2
students will be allowed to check out 1 book each visit, and grade 3-5 students will
be allowed to check out 2 books at one time. All books are due at each library
visit. If your child does not return his/her book on time, he/she will NOT be able
to check out another book until the overdue book is returned. Please make every
effort to ensure the library books are returned on time. We do not charge any
late fees, but if the book is lost or damaged, a replacement fee will be charged and
your child will not be able to check out another book until the fee is paid.
Our library’s catalog is searchable from the district library homepage. Just go to
http://destiny.rcsdk12.org/common/servlet/presenthomeform.do?l2m=Home&tm=
Home&l2m=Home
Then click on 03Nathaniel Rochester Community School. This will take you to our
page. Click on the catalog tab at the top of the page. You can then begin your
search! Have fun exploring the site with your child!
Please return the bottom portion of this page to give your child permission to check
out books from our library.
Thank you,
Mrs. Mauldin

Young Adult Reader’s Permission Slip
Dear NRCS Family of our 7th and 8th grade students,
NRCS strives to include a wide variety of high quality books in our library.
However, some of these books may contain strong language, graphic descriptions,
sexual content, etc. As such, we have a section in the library for “Young Adult”
(YA) books. The YA section contains books that are “rated” for middle school
grades (7th & 8th) have content that may be inappropriate for students in our lower
grades. Some examples of books in the YA section include: The Red Badge of
Courage by Crane, Eragon by Paolini, and the Twilight series by Meyer.
The Young Adult Reader’s Permission Slip is below. By signing and returning this
permission slip, you are authorizing your student to select books from the YA
section. Without the signed permission slip on file your child is free to select from
the rest of the library but NOT the YA section.
If at anytime you would like to revoke this permission slip, send me an email and I
will take of it. Please let me know if you have any questions or concerns.
