Center for Geriatric Health Changing the Approach Olympia Medical Center has changed the approach to healthcare for the geriatric patient. This unique approach includes addressing potential health problems and reviewing the entire family situation including the caregivers -- all at once location. Staying Healthy and Active as We Age Aging brings with it inevitable changes of body and mind, and there are things that we can do to stay healthy and active as we age. At the Center for Geriatric Health each patient is given a comprehensive physical and mental geriatric evaluation and assessment of a patient’s potential health problems. Evaluating Potential Health Problems Chronic Conditions like Anemia & Hypertension Gastrointestinal issues Eye Diseases Arthritis Urinary Incontinence Problems with Walking or Falling Parkinson’s Disease Depression and other Psychological issues Alzheimer’s Disease Early Onset Dementia Confusion and Memory Loss On-going Treatment From A Variety Of Specialists All At The Same Location The comprehensive diagnosis and treatment program at the Center is provided by an array of specialists that also furnish on-going treatment -- all at the same location. With this program there is no need for patients and their caregivers to endure exhausting trips from doctor’s office to doctor’s office to meet with various specialists. Medical Director Robert Wang, MD, PhD The Medical Director of the Center, Robert Wang, MD, PhD, reviews the patients’ current assessment and prior medical reports and determines which physicians are needed to attend to their specific aging issues. Interdisciplinary Team Of Specially-Trained & Experienced Medical Specialists The team of specially-trained and experienced medical specialists includes: Geriatricians Geriatric Psychiatrists Nurse Practitioners Rehabilitation Therapists (Occupational, Physical and Speech Therapists) Nurses Social Workers Nutritionists Cardiologists Ophthalmologists Offering a Wide Selection of Laboratory Tests, Imaging and Other Assessment Tools Since the Olympia Medical Center is adjacent to the Center for Geriatric Health, there is a wide array of laboratory tests, imaging and other assessment tools available to the patient that would not ordinarily be available in a specialist’s office. Identifying Complex Medical Problems The geriatric program features a multidisciplinary staff of medical professionals who work in conjunction with the patient’s primary care physician and Dr. Wang to help identify complex medical problems, address independence issues and make recommendations to patients, their family and loved ones. Considering All the Problems Simultaneously Since there is only one medical chart for the patient at the Center instead of numerous medical records compiled by physicians from different offices, Dr. Wang can consider all the problems simultaneously. This method provides Dr. Wang with an overview of the patient’s functionality and medical status or total condition that is beyond an individual diagnosis. Unlike other Facilities Based on Dr. Wang’s findings, he will arrange for specific members of the interdisciplinary team to see the patient. Unlike other facilities serving the geriatric population, if a patient requires additional services from an imaging center (CT Scans, MRIs, etc.) the patient at the Center for Geriatric Health can be seen across the street at the hospital facility without having to make another appointment for another day. Following Your Appointment Following the patient’s appointment, a nurse or social worker follows up on their progress. To evaluate the on-going treatment, team members meet weekly to assess the patient’s individualized plan and review their improvement. To ensure a seamless continuum of care, Olympia Medical Center provides on-going care coordination with the patient, family members and personal physician. The Center also coordinates care with home health and community agencies.
