JOB DESCRIPTION
TITLE:
Geriatric Care Specialist, Elder Care Counselor
REPORTS TO:
Director of Geriatric Care Management & Caregiver Support
DEPARTMENT:

JOB SUMMARY:
Responsible for direct service to community-living elders and
family caregivers
FLSA CLASSIFICATION: Exempt
RESPONSIBILITIES:
1)
Serves as geriatric care specialist for elders and family caregivers referred for service.
2)
In accordance with established service delivery standards, implements the following care
management procedures:
A.
Completes a comprehensive assessment of clients’ needs.
B.
Establishes a relationship with those clients whose care plan includes on-going
care management services.
C.
Works with referring party, client, family and/or formal and informal support
systems to develop a care plan appropriate for the client.
D.
Mobilizes resources available to ensure implementation of the care plan.
E.
Makes referrals to other agencies and services; provides necessary follow-up and
advocacy to ensure client needs are met.
F.
Documents all care management activities for each client.
G.
Provides consultation to individuals and families to offer support and assistance
with their caregiving responsibilities for older relatives.

Prepares and submits required statistical reports on care management activities.
4)
Participates in development and implementation of Agency’s caregiver support initiative:
 Leads Powerful Tools for Caregivers groups, supports tracking and reporting of
outcomes, outputs.
 Participates in development and delivery of professional presentations:
 Community education seminars, fairs, conferences
 Continuing Education presentations to the professional community
 Provides classroom guest lecturer function per request of participating
universities

Performs responsibilities of Elder Care Consultation and Referral in compliance with
Sheltering Arms Senior Services standards for service delivery:

Provides telephone and/or office consultation with clients according to their
specific elder care concerns

Provides current referral information to clients both over the phone and in written
report form. Visits sites, makes and records observations to enhance referral
information.
C.
Maintains all client, resource, and provider records. Documents completely and
timely all client and client-related contacts.
D.
Tracks and reports outputs and other data as needed by the Agency for accounting,
billing, and evaluation.
E.
Provides backup to other department services as needed.
Attends agency conferences and meetings; participates in in-service training and
seminars.
Maintains effective working relationship with agency personnel to achieve maximum
team approach to service delivery.
QUALIFICATIONS:
1)
Bachelor’s degree in human service field; Social Work, Texas license (LMSW), preferred.
2)
Minimum three years experience that includes services to the elderly.
3)
Knowledge of age-related community resources.
4)
Good computer skills: Word, Excel.
5)
Excellent communication skills, written and verbal: interpersonal, organizational and
public speaking skills.
SPECIFICATIONS:
Physical demands: Work is sedentary. May require light bending or lifting. Work also
involves visiting clients’ homes, which may or may not be handicappedaccessible.
Work environment: Work is performed in a typical office setting. Work also involves visiting
clients’ homes.
