than 60 days from breach discovery
-Must notify Secretary of Health & Human Services (HHS) and prominent
media outlets if breach may involve more than 500 patients
-Must notify next of kin if patients affected are deceased
-Must provide details of the PHI records exposed and what steps are
implemented to help mitigate harm to patient
-Must provide annual notice to Secretary of HHS 60 days after end of calendar
year about breaches that involved less then 500 patients
