Question 1 (denominator) How many unduplicated children/youth under age 18 and in the care of the Contractor during the quarter being reported had at least one Family member, as defined in the contract but excluding parents and siblings, identified by DFPS as appropriate for contact? Outcome: Children maintain connections with siblings Question 4 (numerator) How many of the children/youth reported in question #3 had the minimum number and type of Personal Contacts, as defined in the contract, with each sibling, for each month or portion of a month they were in the care of the Contractor during the Quarter being reported? Question 3 (denominator) How many unduplicated children/youth under age 18 and in the care of the Contractor during the quarter being reported had one or more siblings in foster care? Do not include in count if all siblings were placed together. Outcome: Youth have a regular job Question 5 (numerator) How many unduplicated youth in the care of the Contractor at any time during the fiscal year (i.e., September 1 - August 31) and were age 16 or older on the last day of the period being reported had a Regular Job, as defined in the contract, at any time during the fiscal year? The number reported should be cumulative for the fiscal year. IMPACT data (denominator) Number of children aged 16 or older in care, year to date. Outcome: Youth have a state ID Question 6 (numerator) How many unduplicated youth who were in the care of the Contractor at any time during the fiscal year (i.e., September 1 - August 31) and were age 16 or older on the last day of the period being reported had been issued a driver's license and/or state identification card by the end of the period being reported? The number reported should be cumulative for the IMPACT data (denominator) fiscal year. Number of children aged 16 or older in care, year to date. Outcome: Children participate in placement decisions Question 7 (numerator) How many of the placement changes within the SSCC during the quarter being reported were preceded by at least one discussion with the child/youth to obtain the child/youth’s opinion regarding their placement options, to the extent of the their ability? IMPACT data (denominator) Number of children in the SSCC’s care who experienced a placement change. OC-OK will track this outcome; will not appear on PMET form in Evolv. Outcome: Children/youth participate in court Question 9 (numerator) How many of the hearings in question #8 were attended by the child/youth?
