What is a formative assessment?
 A mid-plan check-in
rating on progress towards each goal
 rating on each standard

 A document that considers all evidence
observations to date- at least two observations prior to
the Formative
 artifacts to date-ensure at least one artifact uploaded by
educator or evaluator for each Standard and goal

 It may be used to change a plan

If there is a significant change in practice, however, this is
not mandatory. The plan may continue until the summative.
Steps of the Formative Assessment
Step 1:
Gather Evidence
• Conduct at least two observations prior
to Formative Assessment to collect evidence
for Standards I and II
• Ensure that at least one artifact has been
uploaded by the educator or evaluator for
each Standard and goal.
Step 2:
Rate goals &
standards
• Rate each goal and Standard
• Write a rationale for each rating that has
a claim supported by at least 1-2
specific pieces of evidence
• The rationale should connect the
performance rating to the rubric.
Steps of the Formative Assessment
Step 3:
Prescriptions
• Any standard rated Needs
Improvement or Unsatisfactory will
require a prescription that includes a
Problem Statement, Evidence Statement,
and Prescription Statement
Step 4:
Comments
• Both the Evaluator and the Educator
have the option of writing comments
on the Formative Assessment.
Steps of the Formative Assessment
Step 5:
Overall Rating
• Evaluators synthesize ratings on Goals and
Standards into an Overall Rating
• Educators must be rated proficient or
better in Standards I and II in order to get an
overall rating of Proficient or Exemplary.
Step 6:
Sign-off
• The Evaluator signs off electronically by typing
name.
• Clicking “Submit Signature” will release
evaluation, allowing the Educator to view the
document.
Step 7: Meet with
Educator
• Evaluators are encouraged to meet with
each Educator to discuss the Formative
Assessment.
