PARENT CONSENT AND WAIVER FORM
As a parent/guardian of the student-participant, I hereby give my consent and acknowledge by
my signature that my son/daughter of Year level and Section
will join the
activities of the Intramurals 2023 to be held at Indahag Elementary School Covered Court,
Indahag, Cagayan de Oro City on December 11, 2023.
I am aware of the usual risks and dangers inherent in the said /event thus, accidents can occur
with or without any fault on either the part of the student or the teachers where the event is taking
place. By allowing my child to participate in this activity, I accept the risk of any untoward
incident and conform to the school rules and regulations that this activity is suitable for my son or
daughter to enhance his/her skills as a student. I have read and understood the terms of this
consent and waiver, and understood well that is binding upon me, my heirs, executors, and
administrators.
