Vernon Russell
Guided Reflection Questions
Opening Questions
How did the simulated experience of Vernon Russell’s case make you feel?
The simulated experience of Vernon Russell’s case went well. I was a bit nervous on not knowing what to
do. But after reading the directions and assessing what is needed for his case made me feel confident
and it also made me feel a lot more comfortable.
Talk about what went well in the scenario.




Taking Vernon Russell’s Vital Signs which included:
Heart rate: 96
Pulse: Present
Blood pressure: 133/79 mm Hg
Respiration: 12
Conscious state: Appropriate
SpO2: 97%.
Temp: 99 F (37.1 C)
I was still able to comprehend and go through the whole simulation/scenario
It was much easier than the perview simulation/scenario
Reflecting on Vernon Russell’s case, were there any actions you would do differently if you were to
repeat this scenario? If so, how would your patient care change?

I wouldn’t do anything differently. I think each step of the simulation was important and
appropriate for the scenario.
Scenario Analysis Questions
S
What priority problem(s) did you identify for Vernon Russell?





Mr. Russell has a history of hypertension
Coronary artery disease
Diabetes mellitus type 2
Smoked over a pack of cigarettes per day for the past 35 years
Mr. Russell does not exercise
 The Scenario Analysis Questions are correlated to the Quality and Safety Education for Nurses (QSEN)
competencies: Patient-Centered Care (PCC), Teamwork and Collaboration (T&C), Evidence-Based Practice
(EBP), Quality Improvement (QI), Safety (S), and Informatics (I). Find more information at:



Dyspahgia
Stroke
Muscle weakness
S/EBP Correlate the abnormal coagulation studies results to Vernon Russell’s current treatment that
has been initiated thus far.

I see that Mr.Russell’s PT level is high. He may need a change of medication to lower his PT
levels. Also, he should stop smoking.
EBP/I What resources are available to the interprofessional health care team to guide the care of
Vernon Russell and/or any stroke patient?

Support groups, physical therapy from therapist, speech evaluations, and much more.
PCC/S Discuss the rationale for Vernon Russell being placed on NPO status?



Vernon Russell was put on nothing by mouth until the speech therapist has completed a bedside
evaluation
Vernon Russell Is at risk for aspiration
He is not able swallow anything without aspirations it into lungs beacause of his stroke/ muscle
weakness (left side hemiplegia)
PCC/S How did you respond to Vernon Russell’s question related to why he needs someone to help
him walk, and what is your rationale for your response?


PCC
It is very important to call for help to prevent injury/falls to keep a client safe.
Utilizing Vernon Russell’s health history, identify factors that predisposed him to having a stroke.




T&C



Factors that predisposed Mr. Russell to having a stroke:
History of hypertension, coronary artery disease, and diabetes mellitus type 2
Also, Mr. Russell smoked over a pack of cigarettes per day for the past 35 years
Mr. Russell also does not exercise
Discuss why the referral for speech therapy is important in Vernon Russell’s care.
The referral for speech therapy is important in Vernon Russell’s care for various reasons such as:
Assessing, diagnosing, and treating disroders concerning speech, language, cognitive
communication, swallowing ability, and other related issues
Receiving quality treatment from a speech therapist can significantly improve Vernon Russell’s
recovery

Concluding Questions
Describe how you would apply the knowledge and skills that you acquired in Vernon Russell’s case to an
actual patient care situation.

The knowledge that I would apply is being able to understand and read what is in the doctors
order. So I don’t make a medical/medication error. If I was to administer a medication to a
patient who is aspirating, the client would be at risk to choking. It is always important to read
carefully and comprehend everything that is in the health record/ doctors order before
administering anything to the client.
