PostoperativelyAdult:IM/IV 50-100mcg q1-2hr prn
Peds: IV 2-3 mcg/kg
Physiological Action: inhibits ascending pain pathways in the CNS, Increasing pain threshold, alters pain
perception by binding to opiate receptors.
Onset of action: IM 7-15min IV 1min
Peak Action: IM 30min IV 3-5min
Duration of action IM 1-2hr IV1/2-1hr
Therapeutic Effect: Controls Moderate to Severe Pain
Excretion: Kidneys, Crosses Placenta, Breastmilk.
Major Side Effects & Toxic Reactions:
Dizziness, Delirium, Euphoria, Sedation, Confusion, Weakness, Dizziness, Seizures
Nursing Implications & Contraindications:
Nursing implications:
Assess the blood lab tests monthly, the risk for toxicity, Elimination, Nutrition, iron loss.
Contraindications:
Hypersensitivity to opiates, Myasthenia gravis
Geriatric Considerations:
Avoid in Older Adults, may cause Ataxia or impaired psychomotor function

Physical Dependence, not to use with alcohol.
