Monitoring and Evaluation
Immediate:
The client, along with his family, is well-informed about the intervention
on the diet and applies the health education provided.
The client is relieved of the shortness of breathing.
The client’s blood pressure shows appropriate reduction.
The client’s cholesterol/triglycerides/LDL level shows appropriate
reduction.
The client’s BMI improves to _____.
The client monitors consumption using the MyFitnessPal App.
The client performs a 30 minute exercise at least thrice a week then
gradually increases length and intensity within appropriate levels.
That being said, the client’s decision of consuming a huge amount of
food, especially high in fat and those high in sodium must be
monitored.
Not immediate:
The client maintains food monitoring using MyFitnessPal App.
The client’s cholesterol is normalized.
The client’s blood pressure is normalized.
The client exercises at least 5 times a week with increased intensity
still within appropriate levels.
The client should be able to keep track of caloric and carbohydrate intake with the
help of MyFitnessPal app. Whereas, complex carbohydrates could be consumed in a
gradual manner. On top of that, the consistent habits of physical activity and
exercises should still be continued.
