I affirm that I have been informed about the retention, tenure, and promotion policies and
procedures of California State University, Chico. I understand that I am required to create and
maintain a dossier of evidence documenting my instructional and professional career and
achievements. Its contents will be forwarded to the Department/Unit Personnel Committee Chair,
upon request, and shall become a part of the Working Personnel Action File while I am under
review. It will be returned to me after the completion of the personnel cycle. I am aware that I am
