Present:
Adrian Amjadi, Rocky Bonura, Robert Brobst, Christie Killduff, Carl Turano and Valerie
Wagner (Recorder).
Absent:
Janice Caldwell, Elizabeth Fernandez, Julienne Gard and Ana Tontcheva.
A. Call to Order: Rocky Bonura called the meeting to order at 1:05 p.m.
1. Minutes: Minutes of July 24, 2013 were approved.
2. Workers’ Compensation: Rocky Bonura reported that there were a total of nine injuries for the
month of August. Six of the injuries were due to Respiratory Care Students being exposed to
scabies at Gardena Memorial Hospital. None of the nine injuries resulted in time away from
work.
3. Office of Safety and Health: Rocky Bonura reported that our insurance consultant will be
conducting two training sessions on Needle Stick Prevention for our Nursing student during the
months of October and November.
Rocky Bonura also reported that he will be discussing the Loss History Report with the Safety
Committee at the October meeting.
4. Safety-related Work Order Report: Robert Brobst report that the following work orders are “inprogress” status: 55131, 55361, 54964, 55644 and 56490s. Work order 54633 with regard to
trimming the trees at the Child Development Center site has been completed. Rocky Bonura
stated that a “Flag” has gone up for him concerning Work Order number, 55131 regarding the
carpet bubbling up in the Student Services Building, since the work order was submitted on June
19th. Has anyone taken a look at this area? This one is a concern for me since there has been
indication from Ms. Kato of a tripping hazard? Who reviews these work orders? Robert Brobst
stated that the supervisors review the work orders, which in this case Michael Blada would be
the supervisor who would have reviewed the comments that were made by Christina Kato
regarding the carpet bubbling and the tripping hazard. Mr. Blada may not have had time to
update the comments to state that he has review it and it is not necessarily a hazard but it is
something that we want to get fixed. Rocky Bonura commented that he can appreciate this but
it does not look good. If we were audited or something would occur between June and now
then we would be liable because we were aware of the hazard, but did not take the steps to
correct it in a timely manner. I would like to have Mr. Blada comment to inform us that
someone has in fact taken a look at the area and make a determination of the severity of the
hazard.
Robert Brobst stated that with regard to work order 55721, he took pictures and recorded a
video concerning the serious safety concerns regarding the weight machines in PE-51. Robert

Brobst reported that when the students are working out using the weight machines in that
room, the weight racks shake. The shake also causes the machines to go in the exact same
direction in reverse every time someone lifts. This causes the machines to be pushed back
about four feet into the vertical Formica curtains, which have become over the years brittle and
have busted apart and now you have sharp edges, exposed metal and wire. The other hazard
that I observed, is a hazard to students. Students are supposed to be able to walk behind the
weight machines and spot an individual lifting the weight, which in this case is not possible. I
presented this safety concern to both Tom Brown and Rory Natividad. Next week the baseball
team will be moving the mats and putting the weight machine into their proper positions before
Facilities can come in and do our part with affixing the weight machines to the concrete flooring.
Rory Natividad will have to find a solution to the old Formica curtains. If someone were to fall
backwards, they would seriously cut themselves.
5. Old Business:
Facilities – Robert Brobst reported that he met with Tom Brown to discuss the “Heat” problem
in the Carpenter, Electric, HVAC and Locksmith Shop areas. In the Locksmith Shop there is a
ceiling ventilator, the locksmith was to have the ventilator either fixed or start using it to vent air
through and into that space to see what difference in temperature the ventilating of the air
would provide before we went any further with this issue.
Natural Sciences – Christie Killduff reported that not only is there an on-going problem in both
LS-109 and LS-113 with the Velcro straps not staying in place on the desks; but we also have a
problem with the desk wheels coming off periodically. The desks are very heavy, making it
difficult to put the wheels back in place. Rocky Bonura suggested that he and Robert Brobst go
take a look at the areas and come up with a solution to this on-going problem.
6. Division Reports:
Facilities – Carl Turano reported that he has a safety concern with regard to a gas valve out on
the campus roadway. The yellow cap that fits over the gas valve should sit flushed, but does not
and sits uneven. This is a tripping hazard because the cap does not sit level on the ground. We
had a cone covering this area for a while, but now the cone has disappeared.
Art Department – Adrian Amjadi reported that his department has begun using the “Rag”
Service with Domestic Linen. Everything is going well so far with the service. Domestic Linen
has been very accommodating for the most part..
We have experienced theft within our building. During the break items were stolen from both
Art-16B and Art-20 and Police reports were filed for both incidents. We also had seven of
twelve power adapters stolen along with some monitors from our computer lab in Art 218. We
are having a huge staffing problem in the computer lab area at the moment, so faculty are
allowing students to go in and out of the computer lab area with no supervision.
I met with
Connie Fitzsimons with regard the adapters because the lab area is crippled without both the

adapters and the monitors. There seems to be a hold up with getting new adapters. I had
submitted a requisition for the equipment and offered purchasing a number of different vendor
options to order from. Rocky Bonura stated that Don Treat was involved and made
recommendations. What was the decision as far as the equipment goes and Do you know if a
Buyer is working on this order? Adrian Amjadi commented by stating that he has to check the
order to find out. The order was just submitted at the end of last week. Rocky Bonura
suggested that Adrian Amjadi follow up to find out the status of this order and notify him of the
status.
During the power outage in July, we had a disabled student on a ventilator in the basement of
the Music Building. Our staff contacted campus police because evacuating the student was out
of our capabilities. Campus Police notified paramedics who were able to evacuate the student
from the basement.
7. Next Meeting: Wednesday, October 23, 2013 at 1:00 p.m. on the 2nd Floor of the Bookstore
Building, Conference Room 253.
