Please download and fill in the registration form, and return it as an e-mail attachment to rnichols@fgcu.edu and
jkellis@fgcu.edu. You may include a copy of this registration form with your check. Registration form must be
submitted by email no later than Wednesday, November 25th, 2015. Registration fees must be postmarked by
Saturday, December 5th, 2015. The registration fee is $12.00 per student, which covers the cost of a tee shirt
designed for the event. Please make a check payable to "FGCU MATH CONTEST" for $12.00 times the total
number of participating students plus an additional $5.00 times the number of coaches shirts and send to:
