Creating Catholic Conditions for Well-Being, Learning and Leading
Outcomes:
1.0 By the end of 2016, there will be an increase in the proportion of students indicating
that they feel accepted by students in the school, as demonstrated by the CCCC School
Climate Survey results.
Outcome Progress Comments
5/31/2015: Continue to share the results of the CCCC survey with the Catholic School Council outlining areas of
need for student well-being. Continue to emphasize school bullying awareness initiatives including: Grade 7 Girls
Well-Being Club, student council organized spirit activities, student improver awards, Smart Start program, Rising
Stars, NPU officer liaison and guest speakers to help educate and inform students. Continue to use a progressive
discipline approach in dealing with all behaviours including Child and Youth Workder and Social Work support as
needed.

Building Collaborative Practices Through Inquiry
Outcomes:
2.0 By the end of 2016, there will be a movement to develop a school culture focused on a
growth mind-set to help support the Board Strategic Plan

5/31/2015: Staff participated in Professional Learning Communities to develop a school inquiry model in the area
of student well-being. They worked to co-construct learning goals and success criteria in various curriculum areas
to build a common understanding of how to address the needs of each learner.

Enhancing Transitional Practices
Outcomes:
3.0 By the end of 2016, there will be an increase in the proportion of Grade 8 students who
are prepared for Secondary School transition through a variety of contexts and active
engagement.
Outcome Progress Comments
5/31/2015: Continued collaborative practices have been established between Secondary and Elementary School
for transitioning Grade 8 students. Elementary staff attend Secondary School transition meetings to establish best
practice. Meeting and organization between elementary and secondary schools occur to organize transitions for
special needs and identified students. Support is provided for all learners through IPRC's, IEP development, school
and parent/guardian relationships, case conferences, ASD and Behaviour Team support, Speech and Language,
Social Work, Psychologist and Child and Youth Worker, Settlement worker and ELL teacher welcoming newcomers.
Welcome to Kindergarten evenings allow for students and parents/guardians to transition into school. Focus will
be on the development of student portfolios as a method of documenting student achievement and personal
goals. (Creating Pathways to Success).

Knowing the Learner through Assessment
Outcomes:
4.0 By the end of 2016, there will be an increase in the proportion of students able to
articulate strategies supporting their learning experiences through the development critical
and creative thinking skills.
Outcome Progress Comments
5/31/2015: Staff will continue to use student profiles as a means of tracking student achievement. Through our
focus on well-being, students will display more confidence to articulate learning goals and success criteria
expectations. Open ended questions and focus on critical thinking are ongoing strategies.

Responding through Effective Instruction
Outcomes:
5.0 By the end of 2016, there will be an increase in the proportion of students improving
their EQAO achievement level between assessment years.
Outcome Progress Comments
5/31/2015: Staff continue to liaise with the Special Assignment Teacher and Grade Level groupings as a means of
enhancing their knowledge and understanding of the collaborative inquiry process and effective teaching
strategies. Student participation in the Tutoring Club for Grade 3 and 6 as well as an in school homework club have
supported student achievement and learning.
