1.1 By June 2016, the proportion of students reporting that they have worries will be
reduced.

6/1/2015: Students have been provided opportunities to explore wellness / mental health strategies to manage
worries; staff and parents have been provided information about mental health and children through the
newsletter, web links and as part of parent engagement evenings.

1.2 CCCC Pillar: By June 2016, the proportion of students who feel excluded will decrease.

6/1/2015: Various retreat opportunities and outreach by staff have been provided to allow students to explore and
deepen their relationships with teachers; various leadership opportunities have been provided to develop sense of
place and influence; CYW continues to support students at risk; two projects - The Lion's Den and Buddy Benches
have been actualized to provide students with strategies and a "place" to work on difficulties.

1.3 By June 2016, an outdoor learning space / classroom will be created to provide students
with an opportunity to pursue inquiry and interaction with the natural environment.

6/1/2015: Outdoor classroom has been created; students have been accessing the space independently during
recess and classes have been using the class as an alternative space for their classrooms.

Building Collaborative Practices Through Inquiry
Outcomes:
2.1 By June 2016, there will be an increase in the proportion of educators who intend to
continue to develop knowledge gained through professional learning opportunities in
collaboration with others.

6/1/2015: All staff have been engaged in professional development in a variety of areas, including math, social
studies, science, inquiry, the arts, FNMI and technology.

Enhancing Transitional Practices
Outcomes:
3.1 By June 2016, all students will be effectively supported in their transitions within the
school and to programs outside of the school.
Outcome Progress Comments
6/1/2015: Family of Schools and Board processes were followed to advocate for the needs of students; meetings
were held to discuss next steps and alternate placements for hard to serve students; ongoing communication with
parents was in place to ensure full disclosure; itinerant resource staff were accessed to provide additional support
for students.

Enhancing Transitional Practices
Outcomes:
3.2 By the end of 2016, students at St. Valentine School will be familiar with high school
options, programs & pathways to determine their appropriate placement for academic
success thus increasing the future graduation rate to the Board target.

6/1/2015: Grade 8 teachers met with secondary school ARD and Guidance departments to discuss any student
needs as they relate to a positive entry into the secondary school setting; grade 8 students and teachers
participated in a "shadow day" whereby the grade 8 students visited the secondary feeder school and were paired
with a grade 9 student ambassador and spent the day following the student to classes and visiting various parts of
the school; ongoing communication with parents to ensure awareness of information evenings and programs
available.

Knowing the Learner through Assessment
Outcomes:
4.1 By June 2016, there will be an increase in successful practices that support knowing the
learner through assessment.
Outcome Progress Comments
6/1/2015: Staff have been involved in work with the SWSI teacher to further develop their questioning,
observation and assessment/evaluation practices to fully know the students in their classes; focus for this school
year has been the development of collaboration and understanding of knowledge building and interdependence.

Knowing the Learner through Assessment
Outcomes:
4.2 By June 2016, there will be an increase in the proportion of educators who increased
their understanding of assessment for and as learning as a result of professional learning
opportunities.

6/1/2015: Assessment / evaluation continues to be a focus during staff meetings; monographs and other
professional resources continue to be shared with staff; during meetings with SWSI teacher, focus on assessment
for, as and of learning has been the focus, especially with "level 2" or at risk students.

Responding through Effective Instruction
Outcomes:
5.1 By June 2016, all teachers will deliver a balanced literacy program and use common
language and common assessment tools with a primary focus on guided reading.
Outcome Progress Comments
6/1/2015: Consistent literacy blocks of approximately 100 minutes per day continues to be a goal and is taking
place a majority of the time, according to primary, junior and intermediate classroom teacher timetables;
consistent use of core resource literacy materials continues to be a focus; making cross-curricular connections has
been a focus and will continue to be a goal for inquiry work next year.

5.2 By June 2016, all teachers to implement three part lesson plans at least three times per
week across all divisions.
Outcome Progress Comments
6/1/2015: All divisions have focused on the 3 part math lesson in division and staff meetings; our SAT has
continued to support various classes in the use of the 3 part lesson in math and in developing rigorous problems
based on the Ontario curriculum; teachers have been working together to focus on consolidation and the coplanning and examination student work using the 3 part lesson model.
