 Fill out and turn in registration form.
 Fill out and return Hold Harmless Agreement and Release forms.
 MUST have payment turned in with forms. NO MONEY-NO TEAM! $5 per person (including coaches) Cash or
check only.
 Must pick up rules and regulations packet for team.
 Practices are held on the teams own time. Be ready to play!
 Teams can have MALE cheerleaders and/or coaches. Cheerleaders DO NOT have to pay to participate. Coaches do!
 Each person participating (including cheerleaders) MUST BE a Kent State Student. A valid ID is required to participate.
Check your schedules! There will be NO REFUNDS if you cannot play or change your mind.
