North Central Thames Foundation School (NCTFS) Individual Placement Description - Luton and Dunstable Hospital NHS Foundation Trust Placement The department The type of work to expect and learning opportunities Location Supervisor(s) Main duties of the placement F2 General Psychiatry/ Community Psychiatry The FY2 in Psychiatry placement is based in Community Mental Health Team based at Calnwood Court, Luton next to Luton and Dunstable Hospital. The placement offers a comprehensive understanding of the working of Mental Health services to support and manage the patients with severe and enduring Mental health conditions as a Multi-Disciplinary Team. This includes input from the Consultant Psychiatrist and supported by the Trained Psychiatric Nurses, Social Workers as well as Occupational Therapists and Clinical psychologists. Calnwood Court hosts a mixture of trainees at any one time, including Specialist and Core Trainees in Psychiatry as well as GP Trainees. The Trainee will be able to participate and observe the working of the Community Mental health Team including assessment, investigations, developing Care Plans and management of patients with Mental health conditions requiring Secondary Mental Health Care. Many of these referrals are from the Primary Care and require Comprehensive Assessment. This placement gives exposure to safe prescribing and Risk Assessment for the management of Mental Health problems. The overall educational objectives of the F1 year are to provide the trainee with the knowledge, skills and attitudes to be able to Take a history and examine a patient’s mental state in the Clinic Identify and synthesise mental health problems Prescribe safely according to BNF guidelines Keep an accurate and relevant medical record Manage time and clinical priorities effectively Communicate effectively with patients, carers and colleagues Use evidence, guidelines and audit to benefit patient care Act in a professional manner at all times Cope with ethical and legal issues which occur during the management of patients with mental health problems Educate patients and carers effectively Become life-long learners and teachers Identify and Manage risk effectively Participate in Multi Disciplinary Team meetings Develop an understanding about the Mental Health Act as well as the principles of ECT Calnwood Court, Calnwood Road, Luton, LU4 0FB Dr.Reshma Inamdar Assessing patients in Out patient clinic to obtain comprehensive History including Medical History Psychiatric History Forensic History Drug and Alcohol History Diagnostic Formulation Mental Status Examination Risk Assessment Investigations including Blood Tests, ECG, Physical Examination Relevant Psychological formulation and Comprehensive care plan development Attend weekly Multi Disciplinary Team meeting Do jobs appropriate to level of training: prescriptions, investigations as well as Physical Examination Liaise with necessary Agencies including General Practitioners Typical working pattern in this placement Completing Assessment Letters and Care Programme Approach Reviews ---------------------------------------------------Overall there is always a specialty trainee and Senior Psychiatrist in the Clinic Weekly: Generally,FY2s are required to run 5 clinics per week as well as Active Participation in one Multi Disciplinary Team Meeting per week Daily Pattern: Complete Clinic and discuss patients/ CPA review with Consultant/ Specialty Trainee in the Clinic The patients include new assessment as well as Recovery Patients. Rota: FY2s are required to participate in 1 in 8 on Call rota with Direct Consultant Supervision Educational Activities: Trainees are supported to attend their Teaching as well as educational activities. Psychiatric Training Session is also led by Consultant / Specialist Registrar for 1 hour every week Employer Whilst trainees will still be employed and paid by the Luton and Dunstable Hospital, information their placement will take place within the South Essex Partnership University NHS Foundation Trust (SEPT), which provides Psychiatric Services across Bedfordshire. SEPT serve a local population of over 650,000 from Luton, Dunstable, Bedfordshire .About 30% of Luton’s population are from cultural and ethnic minority backgrounds and this coupled with the high deprivation found in Luton means our local population display wide range of Mental Health conditions which are more usually associated with those of an inner city area.
