North Central Thames Foundation School (NCTFS) Individual Placement Description - East and North Herts NHS Trust Placement The department The type of work to expect and learning opportunities F2 Intensive Care Medicine Lister Hospital Critical Care Unit Currently 18 Level 2 and 3 beds Beds are used flexibly for level 2 or 3 care depending on need Practical skills Venous access including central venous cannulation Arterial line insertion Larnygocopy and endotracheal intubation Assisting in percutaneous tracheostomies Pleural procedures (aspirates/chest drains) Peritoneal procedures (aspiration/drains) Lumbar punctures Clinical aptitude Highly organized systemic approach to major organ systems enables trainees to understand basic organ function, diagnose a failing trend and institute organ support measures – both ward based and in level 2 and 3 environments Ability to ‘outreach’ or attend peri-arrest and cardiac arrest calls with ICU SpR or ICU Consultant Location Supervisor(s) Main duties of the placement Typical working pattern in this placement Audit/Research Plenty of opportunities to get involved in audit and research Lister Hospital All ICM Consultants – Drs. Prasad, Kambli, Hearn, Carrington, Chilvers, Bates, Patel, Grover, Jamardhakhana, Bramall Attending ward rounds and completing ward round paperwork. Maintaining an up to date ICU patient list Admitting and discharging patients to and from the unit (elective surgical patients post op and emergency admissions). Practical procedures to facilitate advanced organ supportive measures. Death certification. The FYs are the only consistent level of doctor throughout the 4 month period due to frequent SpR and Consultant rotations, hence are expected to contribute to ward rounds and patient discussion, as they help enable continuity of care during these rotations. Weekdays 0800 – 1200 Handover from night staff and daily ward round 1200 – 1600 Ward round jobs e.g. CVC insertion, specialty team patient reviews, organizing scans, talking to relatives, seeing patients being considered for ICU admission, admitting and discharging patients to the ward 1600 – 1700 Recap over the list and update it for the evening and night team This is usually the case for the week except Fridays when often a teaching seminar is held 1230 – 1330 and lunch if often provided On call requirements: Nil and no weekends East and North Herts NHS Trust Employer information It is important to note that this description is a typical example of your placement and may be subject to change.
