Here for Health has now been open for 3 months and has seen 486 individuals. Of 51 questionnaires, 100% of people were extremely likely or likely to recommend Here for Health to their family and friends. Below are some of the comments: - “Friendly, informative advice with proactive approach to helping achieve a regular exercise pathway. - “Fantastic service, providing information on how to live a healthy lifestyle. Should be more of these” -“Able to help set goals. Gained new knowledge which has helped me set a path for a healthy lifestyle” - “Innovative service very helpful and practical” Service development Here for Health is continually looking for ways to reach a wider population to support healthy living. Over the past 2 months we have; -Established our service as part of the patient pathway for the Hypertension Clinic linking with Polly Whitworth, Specialist Nurse Hypertension. - Formed a referral pathway with the Hepatology and Gastroenterology Clinics. In particular a strong relationship has been developed with Dr Jeremy Cobbold, Dr Jane Collier and Mark Ainsworth, Hepatology Research Nurse, who often walk their patients to Here for Health. - Cardiac Pre-Op nurses have been offered the opportunity to bleep Here for Health for their patients to be seen immediately after their pre-op appointment. - Organised weekly pop-up stands across the John Radcliffe site which have covered a range of topics including blood pressure checks, stress management and stop smoking. This has resulted in 74 patient contacts in November. - Developed wider links with a range of community Oxfordshire health and well-being services. As a result we refer service users to a number of community services: Morelife, to attend Weight Watchers or Slimming World on referral Go Active; Get Healthy, Active Body; Healthy Mind for physical activities Generation Games (GG) who have provided Here for Health with their exercise DVD for users Diabetes2gether and other Diabetes support groups Emotional well being support services, Talking Space or Mind can be phoned from Here for Health Drug and alcohol support services; Oasis or Lifeline can be phoned from Here for Health Media Attention: Here for Health recently featured on the BBC Oxford radio breakfast programme and BBC Oxford televised news. The features highlighted the positive forward thinking work by OUH, Oxfordshire County Council and the funders of Here for Health; The Thames Valley NHS England Area Team and The Oxford Radcliffe Charitable funds, to improve the health of the population of Oxfordshire.
