DESCRIPTION OF WORK: This position involves case management compliance for the following: 1) Special Assistance In Home (SA-IH): provides an alternative to Assisted Living 2) Guardianship: facilitates the court process for mentally incapacitated adults 3) Adult Protective Services: screens and evaluates reports of neglect, abuse, or exploitation of disabled adults and provides crisis intervention services as needed 4) assess diagnose, plan and implement treatment plans that provide safe environments for disabled adults and 5) assess customers for emergency assistance through the agency’s crisis and emergency assistance programs 6) assist adults in utilizing appropriate agency services and community partners. Position requires after-hours duty on a rotating basis to provide on-call coverage for Adult Protective Service. Requires the ability to work effectively in a teamwork setting and maintain a cooperative and effective working relationship with families, team members, leadership, community and state partners and the faith community. Applicants must be able to demonstrate intermediate technology skills. Position requires applicant to have a valid driver’s license as well as dependable transportation. . MINIMUM EDUCATION AND EXPERIENCE: Master’s degree in social work from an accredited school of social work; Bachelor’s degree in social work from an accredited school of social work and completion of the Child Welfare Collaborative (Child Welfare positions only); Bachelor’s degree in social work from an accredited school of social work and one year of directly related experience; Master’s degree in counseling in a human services field and one year of social work or counseling; Bachelor’s degree in a human services field from an accredited college or university and two years directly related experience; Bachelor’s degree from an accredited college or university and three years of directly related experience. (Directly related experience is defined as human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.) Degrees must be from an appropriately accredited institution. A certified copy of transcript must be attached in a sealed envelope from the college or university and attached to the application form. PREFERENCES: Prefer applicants with social work experience in a Department of Social Services and Human Services Degree; or social work experience in a human services setting and a Human Service Degree; BSW or MSW Degrees; or a Human Services Related Masters Level Degree.
