CHURCH MATCHING GRANT AGREEMENT

enters into an agreement with Ohio Northern
University to provide student grant aid.

This contract must be received by May 1, prior to the academic year of award.
(Funds may not be available after this date.)
On or before August 10 for the upcoming academic year, we agree to submit grant monies and
certify to the Director of Financial Aid the names of student(s) who are eligible to receive these funds
in accordance with IRS regulations. Ohio Northern will determine the amount of the institutional
match and make adjustments to student aid awards where necessary.
