Welcome & Introduction Martha Jordan, Chair, GHA Community Benefit Committee welcomed members. Congratulations and recognition given to Mark Miller Phoebe Putney Hospital – Healthy Communities Achievement Award winner and to St. Joseph’s/Candler Health System in Savannah, GA 2012 Foster G. McGaw Prize for Excellence in Community Service for its innovative programs to improve the health and well-being of its community. II. Atlanta Regional Collaborative for Health Improvement – Katherine Lawler, Atlanta Regional Commission provided update on the Atlanta Regional Collaborative for Health Improvement. The Playbook link was provided. Action: Continue to follow the work being accomplished by ARCHI. III. IRS CHNA Proposed Rule – a. Eddie Phillips, CPA Draffin & Tucker, LLP presented a summary of the Community Health Needs Assessments for Charitable Hospitals Department of the Treasury Internal Revenue Service 26 CFR Parts 1 and 53 [REG–106499–12] RIN 1545–BL30 April 5, 2013. Proposed rules distributed. b. The American Hospital Association Regulatory Advisory – Proposed Rule on Community Health Needs Assessments and Sanctions for Failing to Meet ACA Requirements for Charitable Hospitals distributed. Action: It was recommended that there was not a need for GHA to submit response. IV. Healthy Communities Institute update – Deryk Van Brunt; Scott Dahl provided update on the Healthy Communities Institute. Scott will now be based in Atlanta and will be covering the Eastern Region. Nine Georgia Hospitals are currently using the system. V. Managing Population Health Megan Moulding, Evidence-Based Aging Services Coordinator, Livable Communities, Georgia Division of Aging Services provided a brief overview of the multiple resources that are available to assist hospitals in the continuum of care efforts. With the understanding that the movement is towards population health, determining the health needs of the community and what resources needed in the community will be imperative. Action: The offer from Megan is to reach out to the Area Agency on Aging. Tap into the Living Well Workshops in GA – evidenced-based program to empower persons and caregivers of persons with any type of chronic condition. VI. 4th Annual Statewide Community Benefit Report – Erin Stewart, Director Public Relations, GHA showed the report to participants. The process will begin again starting in the winter using the 2012 IRS Form 990 Schedule H forms obtained through www.guidestar.org. Action: Begin to send Erin stories and pictures. Categories include: Social and Basic Needs; Health Promotion; Access and Coverage; and Quality of Life. Submit – What is it? Who is it for? Why do it? and What is the impact? VII. VIII. IX. Update from the Department of Public Health (DPH) on secondary data collection & dashboards Report submitted by Gordon Freymann & Robert Attaway provided. DPH continues to work towards provided a user friendly data-base for hospitals to use as they are collecting secondary data for community health needs assessment. Action: Update on DCH Community Health Needs Assessment Grant Award - Dr. Stuart Tedders, Georgia Southern University was invited to attend. Due to scheduling conflict was unable. Feedback from hospitals involved in Grant was noted. Request for sharing – Round Table Discussion- Martha Jordan led a group discussion on Community Health Needs Assessment and Implementation Plan: Lessons Learned. Below are highlights from the discussion. Work with the movers and shakers in the community to get the job done Team work, collaborative Partnership and Executive buy-in Capitalize on what the organization is already doing and link it to the community health needs assessment to Mental Health – access to service and providing free evaluation is meeting need in community Looking at short term and long term goals Using the already existing tools such as the Association for Community Health Improvement tool kit and the Catholic Health Association’s Guide for Planning & Reporting Community Benefit (2012 Edition) Work with local Public Health Department to assist with community building allowing the implementation plan to evolve Implementation plan being submitted with IRS form 990 Use the S.M.A.R.T.E. Approach – Simple/Measurable/Relevant/Time/Evaluate In some areas, public health has not been receptive. If this is occurring please let the Commissioner of the Georgia Department of Public Health (DPH) and State Health Office know so that it may be addressed Interview patients and their families Tap into other community activities such as the school system events Buy - in from School System – begin with Partners in Education; tap into physical education teachers Tap into local Faith Ministries X. XI. Educate CFO to explain impact on budget – o From Budget drainer to External View of hospital service – o From outward view to external view – hospital service to outside need o Cost incurred by providing service ties into needs assessment o Link Services to CHNA and implementation plan o Help prioritize services o Calculate ROI – Specify funds that are going towards services to vulnerable population o Determine Cost over Revenue Over time impact on population health will be determined Working with students – o Calculate number of hours staff is putting towards working with studentsMaintain a paper trail relating to work associated with student contracts and placement of students Reach out to other hospitals in the area o Perfect time to collaborate o Decision makers at table o Prioritize o Determine Impact Pull together community and determine role of hospital Make it regional Next Steps – Recommendations from group – 1. Have Keith Hurle as a break out session at major GHA meeting 2. Continue education sessions on what counts as community benefit 3.
