Aflac’s Personal Accident Indemnity Plan
Policy benefits include:

per day up to a maximum of six treatments
per covered accident.
Initial Accident Hospitalization benefit of

once per calendar year per covered person,
$2,000 if admitted directly to ICU.
Accident Hospital Confinement benefits of
$250 per day: and additional benefit of $400
per day if person is confined in an intensive
care unit (maximum of 15 days per covered
accident for ICU)
