Accident follow-up treatment benefit of $35 per day up to a maximum of six treatments per covered accident. Initial Accident Hospitalization benefit of $1,000 for a 24 hour overnight stay payable once per calendar year per covered person, $2,000 if admitted directly to ICU. Definition of Total Disability: “A continuing inability to perform the material and substantial duties of your full-time job” you must also be under the care and attendance of a physician for your condition. Pre-Existing Condition Limitation: “A sickness or an injury, for which medical advice or treatment was recommended by a physician within the 12-month period before the effective date of the coverage. Pre existing conditions that occur more than 12 months after the effective date of coverage will be covered.
