PERFORMANCE BASED CONTRACT GUIDELINES
1. WHAT IS THE PURPOSE OF THE SERVICE?
(Describe in detail any services to be provided or materials to be purchased)
The purpose of CareerVisions’ services is to build the capacity of YPS to increase student
achievement and socioeconomic integration through professional development and afterschool
programming that supports the integration of the Community Change and CareerVisions Models.
2. AMOUNT OF SERVICE
(set forth all quantities and/or amounts of time required to be devoted to the contract and describe where services
are to be provided as specified in Appendix A)
During the implementation period, CareerVisions, LTD. will provide its unique day
school/advisory or afterschool program known as “The CareerVisions Institute.” “The
CareerVisions Institute” focuses “The CareerVisions Institute” will serve as labs for the
integration of career explorations, community engagement/social activism and STEAM
activities. The CareerVisions Institute supports the Board of Regents Reform Agenda with its
focus on preparing students to be career-ready.
CHA:
 Day school professional development for the integration of the CareerVisions Model
into social studies, ELA and/or special education classes @ $450/session (24 sessions)
= $10,800.
YMS:
 Day school professional development for the integration of the CareerVisions Model
into social studies, ELA and/or special education classes @ $450/session (20 sessions
per teacher per semester) = $9,000;
3. WHO IS SERVED?
(Describe whether services are to be provided directly to students, to staff, etc.)
School teachers, administrators and partners will receive the professional development
services of CareerVisions, LTD.
4. WHO WILL PROVIDE SERVICES?
(If individual providers are contemplated, set forth the names and qualifications of the service providers)
CareerVisions, Ltd. will provide the aforementioned services. CareerVisions, Ltd. provides
youth programs, professional development, fund development, strategic planning and
management services to community-based organizations, local educational agencies, and
institutions of higher learning. These services revolve around the support and adaptation of the
Community Change and CareerVisions program models. These are approaches for community
organizing and career explorations, respectively. CareerVisions, Ltd. also offers workshops
for parents/guardians on a wide array of parenting, educational, societal, economic
development, and health issues.
CareerVisions, LTD has been subcontracted to manage programs in two elementary schools,
twenty middle and high schools and four community centers throughout New York City. Our
clients range from non-profit organizations, private companies, school districts and colleges in
New York City and in Westchester County. They include:

5. WHAT WILL BE COMMUNICATED TO DISTRICT PERSONNEL, PARENTS,
OTHERS ABOUT PROGRESS AND RESULTS OF THE SERVICES?
Student afterschool attendance as well as participant attendance and feedback will be reported
back to the District.
6. HOW WILL THE QUALITY BE JUDGED?
(Set forth the method which will be used to evaluate contractor’s performance)
The contractor’s performance will be based on the following:
 Student enrollment and attendance in the afterschool program.
 Number of sessions delivered.
 Attendance at each session.
 Meeting sign-in sheets
 Phone call summaries
 Teacher lesson plans from collaborative planning sessions.
 Observation reports.
7. PERSONS RESPONSIBLE FOR ADMINISTERING THE CONTRACT.
(There must be a single Board administrative employee identified as the person responsible. This person will also
be responsible for signing off on contract payments)

These services are pursuant to the NYSED Socioeconomic Integration Program grant that was awarded
to YPS early this year. The grant requires a range of services to increase student achievement and
socioeconomic integration, including professional development and afterschool programming.
