BeHIP (Better Health Incentive Program) Your health is important to you, your family and your friends. Your health and well-being are also important to the Lycoming County School Districts and Penn College. As your employer, we recognize the value of investing in a comprehensive Wellness Program which promotes healthy lifestyles and builds a culture of wellness through educational opportunities, wellness activities, and support for self-improvement through healthy lifestyle choices. The goals of our wellness program are to decrease the risk of disease and to enhance the quality of life of our employees. For this reason, we are offering insured employees and their spouses participation in BeHIP! (Better Health Incentive Program). All information collected is kept with either Blue Cross NEPA or with the Health and Wellness Coordinator. No information is ever shared with your employer and information is kept confidential! The BeHIP program will start on August 1st, 2014 and run through July 31st, 2015. There are two parts to this program in order to earn your $150 reimbursement. 1. You must earn 450 points by completing certain activities. (Most of the activities you are doing already earn you points.) Participation in this program is completely voluntary. All activities must be completed by July 31st, 2015. (See next page for point values of activities.) 2. Once you have reached your 450 points, you will need to turn in receipts for health and wellness items you have purchased throughout the entire year. A list of examples for those items are available at the end of this document. All information can be found at http://health.iu17.org *Please keep track of all points in the online tracking system at shapeuptracker.org. You must register in the system if you did not participate in the Employee Wellness Challenge last spring of the wellness program last year. Please use your email address from school and your school password when registering. Unless you are a PCT employee, then please use the email that you want. Then you must enroll in the 2014-15 BeHIP program. All points values are already loaded in the system. Please put quantity of 1.) The steps to earn points are outlined below: 1. Health Risk Assessment (HRA) You must complete an online HRA through Blue Health solutions. HRA’s must be completed by Jan 31st, 2015. Point value-100 2. Biometric Health Screening Must be completed by February 28th! You must participate in the free health screening offered through the workplace which includes height, weight, Blood Pressure, BMI, and a Cholestech finger stick which includes a full cholesterol panel and glucose results. (All tests must be performed in order for you to get points.) Point value=100 3. Health Coaching- (1-866-262-4764) a. Coaching for a medical condition, lifestyle management or disease management- Must complete a minimum of 4 calls through Blue Health Solutions. Calls must be 30 days apart. Point value=100 b. Calls must be 30 days apart. 4. Proof of annual physical exam through a physician’s office (Insurance will pay for only one physical yearly). Must fill out annual physician physical exam form and turn it into Carmen Terry, Health and Wellness Coordinator or you can turn in the Blue Cross NEPA physical form to BCNEPA. Point Value = 100 5. Declaration of a Primary Care Physician-Verification form must be turned into the health and wellness coordinator. Point Value=-25 points 6. Completion of a Personal Health Record- Verification form must be turned into the health and wellness coordinator. This is found on the bcnepa.com website, under the health and wellness tab. Click on My Health Solutions, Click on Health Record and add or edit information. Point Value=-25 points 7. Non-Tobacco User Pledge-Verification form will be mailed to you by BCNEPA or can be found on the health and wellness website. Point value=25 8. Weight Management program- Point value=100 a. Weight Watchers at Work or Community meeting, must show proof of active member. Minimum attendance-14 meetings b. Blue Health Solutions weight management you must complete 6 calls with a registered dietician. Recommended that calls are 30 days apart. c. Other certified weight management programs must be able to show proof of consistent attendance or participation and progress of weight loss. Verification must be turned into Carmen Terry, Health and Wellness Coordinator by August 1, 2014 for point value to be awarded. 9. Enrollment on the Spirit of Women Program through SHS. Point Value=25 for enrollment only Participating in a monthly event or Signature Event= 10 point each (Maximum number of 3 events for 30 points) Total points allowed in this category are 55. 10. Participating in a 5k Walk/Run Race or bike race. Point Value= 20 (Maximum number of points in this category is 80.) You must be able to show proof of completing the event. Website chip times will work or race director/organizer signature/date on numbered bib. 11. Annual Flu shot-Verification form must be turned into Carmen Terry, Point Value=20 12. Enrollment and completion of a Tobacco cessation program-Participation in the Blue Health solutions program must have 6 verified calls which are recommended to be 30 days apart. Point Value=75 13. Enrollment and completion of BCNEPA action plans or Self- leadership module. Point Value = 10 for each completed action plan or module. Action Plans are 9 week online programs that you can complete weekly. They include 1. Depression 7. Heart Disease Prevention 2. Diabetes Management 8. Physical Activity 3. Diabetes Prevention 9. Quitting smoking 4. Financial Wellness 10. Risky Drinking 5. Healthy Eating 11. Stress Management 6. Heart Disease Management 12. Weight Management 14. Completion of a Diabetes Academy sponsored by LCIC Health and Wellness Program Point Value=50 15. No text while Driving Pledge- All Forms must be turned into Carmen Terry, the Health and Wellness Coordinator. Point Value= 25 16. Annual Dental Preventative Exam-All Delta Dental insured employees do not need verification form. If you are not enrolled in Delta Dental through the school insurance you must fill out the dental verification form and turn it into Carmen Terry. Pt value= 25 points for each preventative cleaning (maximum of 50 points) 17. Annual Preventative Vision Exam-Verification form must be turned into Carmen Terry, Health and Wellness Coordinator. Pt Value= 25 points (Only one visit allowed for points per incentive year) 18. Educational session- Sponsored by LCIC Health and Wellness. Dates and times TBD during the year. Pt Value=10 pts per session. 19. Age appropriate Preventative screenings: Verification form found on website. Annual Mammogram-25 points Annual Ob/Gyn exam-25 points Annual Prostrate exam-25 points Annual Colonoscopy-25 points Annual Dermatology exam-25 point 20. If you meet 3 out of 4 benchmarks outlined below please provide us with a verification statement from your physician or you may use your verification through your health screening, provided that you have signed the waiver form. Verification cards will be available on the health and wellness website. Please understand that you will receive only one check during the year so if you would like the full amount of $150, please wait until you have receipts for the total amount. You may turn in receipts at any time, but if it is less than the $150 you will only receive a check for the amount you turn in.
