Essentials Academy of Cosmetology was established in 2011. We offer individual or combination of

Cosmetology, instructor, and Manicuring, training. The school is owned by Melissa Gibbens. Melissa has over twenty-five (25) years of Cosmetology and Manicuring experience as a salon owner, as well as a school owner. The school’s main emphasis is on modern techniques which include but are not limited to: haircutting, coloring, specialty coloring, corrective coloring, permanent waving, styling thermal curling iron, acrylics, and nail enhancement, facials, and waxing. You are trained at your own pace, one-on-one adult education. We offer advanced training and, as a matter of routine invite guest artists to conduct specialty classes.

Essentials Academy of Cosmetology has set up an organization of different positions each is created to assure a smooth operation of the Academy. We have always emphasized “Team Work” as the concept, rather than the hierarchical concept, of management. Each position is only in relation to the load of responsibility imposed upon the person occupying that position:

Melissa provides financial and administrative oversight. She maintains inventory, orders textbooks, student kits, and working supplies. She conducts theory and practical classes. She assumes responsibility for recruiting, enrollment, re-enrollment, staffing, counseling and guidance to both staff and students.

Background: Missouri Cosmetologist/Manicurist/Instructor. Graduated from Missouri College of

Cosmetology in 1984. Matrix Color Specialist. Melissa brings a vast knowledge of business administration, sales, and marketing as a Missouri Real Estate Broker and business owner.

Ashely Moore-Director of Education/Instructor

Ashley is under the direct supervision of the director (s), and assumes the responsibility for education and counseling. She oversees curriculum education activities and class schedules. Ashley conducts academic classes, educates and prepares students in all areas to ensure a passing ration of the highest percent. She assists with inventory, and maintains student academic records and progress reports.

Background: Ashely is in charge while the owner is off the premises. Ashley has been personally trained in the salon by the owner\executive director. Ashley is a perfect addition to teaching the innovative methods that Essentials academy of cosmetology uses. Ashley has owned and operated her own salon; and now enjoys sharing her practical work and business knowledge with our students.

All of Essentials instructors are required to meet the Missouri State board of cosmetology in continuing education (C.E.) Standards, but not less than 12-clock hours of such activity each year, per instructor.

Methodology. Each instructor reviews a written performance evaluation at least once every 12 months.

The evaluation includes, at a minimum, teaching preparation and effectiveness.

Background: Mrs. Rowe is responsible for overseeing all aspects of Essentials Academy financial book keeping. She is in charge of accounts receivable and reconciles all monthly balance statements. In addition she advises with all record keeping. Mrs. Rowe brings over 50 years of experience in management and book keeping. She has successfully managed many physicians’ offices, and consulted and done book keeping for numerous small businesses. She has owned her own business B&B book keeping for since 1985.

There is adequate parking for students and clients. We ask that students reserve the front spaces for handi-cap parking and clients. Please do not leave your vehicle after 10:00 PM without permission any vehicles left are subject to being towed.

GENERAL ADMISSIONS/TUITION

TRANSFER STUDENTS

Any applicant who has prior training in cosmetology, or Manicuring and wishes to complete training in this school, must first fulfill the admissions requirements. Essentials Academy of Cosmetology has no maximum number of clock hours they will except as a transfer student. Students who transfer within the state must submit a valid transcript of hours prior to beginning school, or hours must be approved by the Missouri State Board of Cosmetology. Transcripts will not be accepted after you have enrolled unless noted in the contract. The students will be evaluated and placed in the proper category, theory hours that are not part of a valid transcript may not be accepted, and a written test will be given to determine theory progress.

This school does not recruit students who are currently registered, enrolled, or attending another school.

TRANSCRIPT POLICY

Essentials academy of cosmetology may accept clock hours from another institution once the following conditions are met: o A transcript accepted by the Missouri State board of cosmetology showing the number of accumulated hours that are transferable for the course. o The school may require practical and written evaluation to determine hours students will be given.

Each student is responsible for their tuition, fees, and any charges incurred while attending this school.

Students exceeding their contract date will be charged a fee of up to $250 for each week over contract until completion of program. Transfer students will be charged $6.67 per clock hour. We will be happy to work with you.

School Objectives

COURSE OBJECTIVES

Per Missouri State board of cosmetology, we seek to prepare students for at least a beginning/entrylevel employment. Please refer to the curriculum section of this catalog for more detailed information.

Cosmetology entails teaching the art of cutting, curling, coloring and designing the hair, as well as caring for the skin and Manicuring/pedicuring. The program is 1500 clock hours; approximately 11 months at 36 hours per week, plus a passing grade of 75% on the exams.

Near completion of clock hours, each student will be given a mock board. A minimum of two instructors will administer the practical exam. The practical exam will be given in the exact format that is required for testing by the Missouri State board of cosmetology. There will also be 125 question written exam that will comprise of all areas of cosmetology and infection control. Both portions of the exam must be passed with a minimum score of 75%.

At the time that all testing is complete, and tuition and other fees have been paid in full to the academy, an exit survey has been completed, and all required clock hours have been obtained; students hours will be turned into the Missouri State board of cosmetology and a completion of hour’s certificate will be awarded to the student.

Instructor course objective is the presentation of the skills necessary to educate the above mentioned courses.

This program consists of 600 clock hours. In order to take the Instructor course, the applicant must hold a current license as a Cosmetologist, Manicurist or Esthetician in the state of Missouri, or a state that accepts reciprocity with the state of Missouri. The program is approximately four months long at 36 hours per week. Near completion of clock hours, each student will be given a mock board. A minimum of two instructors will administer the practical exam. The practical exam will be given in the exact format that is required for testing by the Missouri State Board of Cosmetology. There will also be a 125 question written exam that will be comprised of all areas of cosmetology and infection control, and teaching methodology. Both portions of the exam must be passed with a minimum score of 75%. At the time all testing is complete, tuition and other fees have been paid in full to the academy, an exit survey has been completed, and all required hours have been obtained, student’s hours will be turned into the

STUDENT GUIDLINESS

PERSONAL CONDUCT AND APPEARANCE

All students will be required to maintain a professional attitude and professional conduct at all times.

Misuse of policy rules and regulations may be grounds for termination. Foul language will not be tolerated. All services provided, whether to a student or public, must go through the front desk prior to the start. Black or white slacks, skirts, shorts (as long as are no more than 6 inches above the knee) with a white or black shirt or an Essentials academy of cosmetology t-shirt, is considered proper attire. Please make sure your shoulders are covered, feet are covered, and your clothing is clean. Jeans may be worn on Saturdays only. It is encouraged to maintain a professional appearance at all times as we have many salon owners that stop by the academy to meet our students. Aprons or smocks need to be worn when on the student salon floor. It is your responsibility make sure they are kept clean. Hair, nails, and makeup should be done before you arrive at school! Students not in compliance with dress code will be asked to clock out and go home to change.

LUNCH AND BREAKS

It is the student’s responsibility to make sure a lunch is taken any day they are in attendance for five or more hours. A (1) one hour lunch break should be taken during the day. When providing services on the student salon floor it is the student’s responsibility to notify an instructor that they need to take a lunch break. Please try to notify instructor before beginning a service, if at all possible, if you have not had a lunch break.

VISOTRS/CELL PHONES/FAMILY SERVICES/MISC.

Please keep personal visits to a minimum. Phone calls on the school phone should be kept short as this is a business line. If you bring your cell phone to school, it must stay in the student lounge not on the student salon floor and in the classroom. We encourage you to bring your families in for salon services; however, we expect them to pay the usual prices. If it is someone you are financially responsible for, then there is only a charge for the product used. All food and drinks should be kept in the break room not that classroom or salon floor.

LOST/STOLEN/ABANDED ITEMS

1.

Items left on school premises more than 30 days will be considered as stored, and storage fees will apply. Items left more than six months will be considered abandoned and become the property of the school.

The school is not responsible for lost or stolen items while on school premises. Students are advised to label their items.

CLASSES AND SCHEDULE

You set your schedule at the time of contract signing. Schedules can include no less than 16 hours per week unless specially approved by the executive director. Your theory class time and

A Special lab class may be conducted any day by a guest artist, or instructor you will need to be flexible in your schedule availability.

All training classes will be in English unless otherwise specified. Instructor boards are in English only.

When creating your schedule, determine your concentration is your career choice.

ACADEMIC AND PRACTICAL LEARNING METHODS

The school has developed comprehensive curriculum for each course of study which includes requirements for study, weekly class schedules, detailed lesson plans, handouts and project sheets, visual aids and support materials. Each course of study meets the standards established by the state regulatory agency, as applicable. Educators are trained in a variety of teaching methods which are incorporated into the instruction of each course. STATE REQUIRMENTS

The state’s basic requirements are completion of 1500 clock hours in cosmetology, 400 hours in

Manicuring and 600 in Instructor. Students shall meet the minimum required units, regardless of hours, for eligibility to receive a certificate of completion. A grade of 75% will be earned only after requirements of both the state of Missouri and Essentials Academy of Cosmetology are fulfilled.

Required hours are to be completed on school premises. Time will not be given for work outside of the school and is illegal without proper licensure or permission in advance to the state of Missouri. Students will follow all state laws and regulations. State law strictly forbids the practice of cosmetology/esthetics/Manicuring without a license. Students must be under the supervision of a licensee instructor. Violators will be reported to the Missouri State board of cosmetology.

SATISFACTORY ACADEMIC PROGRESS

The Satisfactory Academic Progress Policy is consistently applied to all students enrolled at the school. It is printed in the catalog to ensure that all students receive a copy prior to enrollment. The policy complies with the guidelines established by the National Accrediting Commission of Career Arts and Sciences

(NACCAS) and the federal regulations established by the United States Department of Education.

EVALUATION PERIODS

Students are evaluated for Satisfactory Academic Progress as follows:

Cosmetology 450, 900, 1350, 1500 clocked (actual) hours

Instructor 300, 600 clocked (actual) hours

*Transfer Students- Midpoint of the contracted hours or the established evaluation periods, whichever comes first.

Evaluations will determine if the student has met the minimum requirements for satisfactory academic progress. The frequency of evaluations ensures that students have had at least one evaluation by midpoint in the course.

ATTENDANCE PROGRESS EVALUATIONS

Students are required to attend a minimum of 67% of the hours possible based on the applicable attendance schedule in order to be considered maintaining satisfactory attendance progress. Evaluations are conducted at the end of each evaluation period to determine if the student has met the minimum requirements. The attendance percentage is determined by dividing the total hours accrued by the total number of hours scheduled. At the end of each evaluation period, the school will determine if the student has maintained at least 67% cumulative attendance since the beginning of the course which indicates that, given the same attendance rate, the student will graduate within the maximum time frame allowed.

ACADEMIC PROGRESS EVALUATIONS

The qualitative element used to determine academic progress is a reasonable system of grades as determined by assigned academic learning. Students are assigned academic learning and a minimum number of practical experiences. Academic learning is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion only when rated as satisfactory or better. If the performance does not meet satisfactory requirements, it is not counted and the performance must be repeated. At least two comprehensive practical skills evaluations will be conducted during the course of study. Practical skills are evaluated according to text procedures and set forth in practical skills evaluation criteria adopted by the school. Students must maintain a written grade average of 75% and pass a FINAL written and practical exam prior to graduation. Students must make up failed or missed tests and incomplete assignments. Numerical grades are considered according to the following scale:

93 - 100 EXCELLENT

75 - 84 SATISFACTORY

85 - 92 VERY GOOD

74 and BELOW UNSATISFACTORY

DETERMINATION OF PROGRESS STATUS

Students meeting the minimum requirements for academics and attendance at the evaluation point are considered to be making satisfactory academic progress until the next scheduled evaluation. Students will receive a hard-copy of their Satisfactory Academic Progress Determination at the time of each of the evaluations. Students not maintaining Satisfactory Academic Progress are subject to sanctions.

WARNING

Students who fail to meet minimum requirements for attendance or academic progress are placed on warning and considered to be making satisfactory academic progress while during the warning period.

The student will be advised in writing on the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the warning period, the student has still not met both the attendance and academic requirements, he/she may be placed on probation.

PROBATION

Students who fail to meet minimum requirements for attendance or academic progress after the warning period will first be placed on probation and considered to be making satisfactory academic progress while during the probationary period, if the student appeals the decision, and prevails upon appeal.

Additionally, only students who have the ability to meet the Satisfactory Academic Progress Policy standards by the end of the evaluation period may be placed on probation. Students placed on an academic plan must be able to meet requirements set forth in the academic plan by the end of the next evaluation period. Students who are progressing according to their specific academic plan will be considered making Satisfactory Academic Progress. The student will be advised in writing of the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the probationary period, the student has still not met both the attendance and academic requirements required for

RE-ESTABLISHMENT OF SATISFACTORY ACADEMIC PROGRESS

Students may re-establish satisfactory academic progress, by meeting minimum attendance and academic requirements by the end of the warning or probationary period.

INTERRUPTIONS, COURSE INCOMPLETES, WITHDRAWALS

Students who withdraw prior to completion of the course and wish to re-enroll will return in the same satisfactory academic progress status as at the time of withdrawal.

APPEAL PROCEDURE

If a student is determined to not be making satisfactory academic progress, the student may appeal the determination within ten calendar days. Reasons for which students may appeal a negative progress determination include death of a relative, an injury or illness of the student, or any other allowable special or mitigating circumstance. The student must submit a written appeal to the school on the designated form describing why they failed to meet satisfactory academic progress standards, along with supporting documentation of the reasons why the determination should be reversed. This information should include what has changed about the student’s situation that will allow them to achieve Satisfactory Academic

Progress by the next evaluation point. Appeal documents will be reviewed and a decision will be made and reported to the student within 30 calendar days. The appeal and decision documents will be retained in the student file. If the student prevails upon appeal, the satisfactory academic progress determination will be reversed.

NONCREDIT, REMEDIAL COURSES, REPETITIONS

Noncredit, remedial courses, and repetitions do not apply to this institution. Therefore, these items have no effect upon the school's satisfactory academic progress standards.

TRANSFER HOURS

With regard to Satisfactory Academic Progress, a student’s transfer hours will be counted as both attempted and earned hours for the purpose of determining when the allowable maximum time frame has been exhausted.

EXTRA-INTRUCTIONAL CHARGES

Each course/program has been scheduled for completion within an allotted time frame. A grace period of approximately 10% has been added to the calculated completion date for each program. It is not realistic to expect to receive an education for free. The school has reserved space, equipment, and licensed instructors for each student and course/program. If a student does not graduate within the contract period, additional training will be billed at the rate of $250 per week, or any part thereof, payable

COURSE REPETITION

If a student needs to repeat a course, for any reason they may be subject to the current RE-ENTRY fee. If they are accepted and were making satisfactory progress as defined in the satisfactory academic progress policy section, at that point of withdrawal, they will be considered making satisfactory progress at the point of RE-ENTRY and will RE-ENTER in the same status as in place as time of departure. If the student did not meet the minimum requirements for satisfactory progress of the time of withdrawal, they may be placed on probation until the next scheduled evaluation point and all elements of Satisfactory Academic

Policy are meet, if accepted for re-enrollment.

SALON FLOOR AND STUDENT SERVICES

As a rule clients come first; however, here at essentials we understand that students desire to have student services. Students may receive student services when we have a slow time, or with the mission of the instructor, you may perform student services on other students or vice versa. If you are having a

Chemical Service you must pay in advance for product you are using. In Booking Salon Floor services instructors will make their best effort in distributing clients fairly among students. If a client request a specific student then that request will be honored.

SUBSTITUTE INSTRUCTORS

A list of qualified substitutes will be available. All substitute instructors will attend staff meetings, and an orientation, complete routine employment paperwork, and provide the following documents: a current

Missouri instructor’s license and documentation of current continuing education classes. The owner,

Melissa Gibbens, is a qualified instructor in both cosmetology and Manicuring. She will be readily available in most cases that a substitute is needed.

CERTIFICATE OF COMPETION

Upon satisfactory completion of the required hours, all fees and charges are paid, and all paperwork is completed, a certificate of completion is issued stating that the student has completed adult education as prescribed by the

Missouri State Board of Cosmetology. We do not guarantee the successful completion or the passing, of the Missouri

State Board of Cosmetology examinations. Students must put

ATTENDANCE

Taking time off could cause you to go beyond your enrollment agreement contract date, resulting in charges per that agreement. If you’re going to be absent or late you must call the school, or an instructor.

Students absent for more than 14 days will be terminated according to school policy unless otherwise excused.

PERSONAL LEAVE OF ABSENCE

Essentials Academy of Cosmetology does not offer leave of absences for any reason.

Withdrawal/Termination Policy

WITHDRAWL FROM INCOMPLETE COURSE

Any student who withdraws from their contract and course, or fails to complete their training, will have a notice placed in their student file as to progress at the point of withdrawal. They will become subject to all fees as prescribed in the refund policy/cancellation settlement section.

TERMINATION

Below is a list of some of the most common reasons for termination:

The use of alcohol and/or drugs on school premises.

Padding of operational sheets, i.e.: Daily and or monthly worksheets

Insubordination towards clients, fellow students, instructors or any staff member.

Disruption of class.

Obtaining credits by dishonest means.

Failure to maintain a satisfactory attendance.

Breach of contract.

Not keeping up with assignments.

Missing scheduled monthly payments.

Practicing cosmetology away from the campus without being under the supervision of an instructor and permission from the Missouri State board of cosmetology as a field trip.

RE-ENTRY/ RE-ENROLLMENT AFTER TERMINATION

After being terminated for any reason, a student may be re-admitted at the same satisfactory progress status as the time of termination or withdrawal if the following conditions are met:

The student must satisfy all charges incurred prior to termination, including charges computed and on policy/cancellation settlement section

The student will be subject to the current RE-ENTRY fee.

The student will re-register, and sign a new enrollment agreement with re-enrollment date. This means that he/she must pay current cost of tuition and fees and must also pay the registration fee again; however, the previous hours earned shall be used as credit in computing new charges.

The student agrees to be placed on probationary status for 30 days or until the next evaluation period, after which the student’s grades, attitude, attendance, and general progress will be reviewed and evaluated in order to be considered for ongoing acceptance. If the student has not satisfactorily met the requirements for acceptance, termination will become final.

SPECIAL EXEMPTIONS WAIVERS

If for any reason a student is unable to meet their contractual obligations they may request a special exemption waiver. This waiver allows the director to make exceptions to written policy, based on extenuating circumstances. Failure to obtain a special exemption waiver may result in actions being taken against the student.

EXTENUATING CIRCUMSTANCES

Situations sometimes arise where a student is unable to abide their contractual obligations. Some of those situations will be due to extenuating circumstances. In dealing with these situations, such circumstances may be taken into consideration through the use of a special exemption waiver. Below are some examples of extenuating circumstance, which may cause the student to be unable to meet their contractual obligations:

Hospitalization of students or serious injury or illness.

Doctor’s orders to remain in effect for a specific number of days beyond five. Medical release to return to school is required

Circumstances occurring beyond the student’s control.

Child care issues

Employment obligations

REFUND/CANCELLATION POLICY

For applicants who cancel enrollment or students who withdraw from enrollment a fair and equitable settlement will apply. The following policy will apply to all terminations for any reason, by either party, including student decision, course or program cancellation, or school closure.

Any monies due the applicant or students shall be refunded within 45 days of official cancellation or withdrawal.

Official cancellation or withdrawal shall occur on the earlier of the dates that:

1.

An applicant is not accepted by the school. The applicant shall be entitled to a refund of all monies paid.

2.

A student (or in the case of a student under legal age, his/her parent or guardian) cancels his/her enrollment in writing within three business days of signing the enrollment agreement. In this case all monies collected by the school shall be refunded, regardless of whether or not the student has actually started classes.

3.

A student cancels his/her enrollment after three business days of signing the contract but prior to starting classes. In these cases he/she shall be entitled to a refund of all monies paid to the school less the registration fee in the amount of $100.

4.

A student notifies the institution of his/her withdrawal in writing.

5.

A student is expelled by the school. (Unofficial withdrawals will be determined by the institution by monitoring attendance at least every 30 days.)

In type 2, 3, 4 or 5, official cancellations or withdrawals, the cancellation date will be determined by the postmark on the written notification, or the date said notification is delivered to the school administrator or owner in person.

For students who enroll and begin classes but withdraw prior to course completion (after three business days of signing the contract), the following schedule of tuition earned by the school applies. All refunds are based on scheduled hours:

All refunds will be calculated based on the students last date of attendance. Any monies due a student who withdraws shall be refunded within 45 days of a determination that a student has withdrawn, whether officially or unofficially. In the case of disabling illness or injury, death in the student's immediate family or other documented mitigating circumstances, a reasonable and fair refund settlement will be made. If permanently closed or no longer offering instruction after a student has enrolled, the school will provide a pro rata refund of tuition to the student OR provide course completion through a pre-arranged teach out agreement with another institution. If the course is canceled subsequent to a student's enrollment, the school will either provide a full refund of all monies paid or completion of the course at a later time.

Students who withdraw or terminate prior to course completion are charged a cancellation or administrative fee of $150.00. This refund policy applies to tuition and fees charged in the enrollment agreement. Other miscellaneous charges the student may have incurred at the institution (EG: extra kit materials, books, products, unreturned school property, etc.) will be calculated separately at the time of withdrawal. All fees are identified in the catalog and in this enrollment agreement.

UNOFFICIAL WITHDRAWL

Students who have 14 days of consecutive absence without contacting the school will be considered withdrawn. Clock hours will be checked on a minimum of every 30 days. If a student has been officially withdrawn refund policy will apply.

SAFTEY AND EVACUATION POLICY

BASIC REQUIREMENTS FOR A SAFE WORKPLACE

1. Proper Ventilation: Some fumes can be harmful.

2. Proper Use of Flammables: Read labels and always follow precautions.

3. Designated smoking areas: Never smoke or permit clients to smoke while being served.

4. Avoid other sources of open flames.

5. Safe Product Storage: Store products in closed containers and prevent spills or leakage. Store in adequately ventilated area and in moderate temperature.

6. Protection during application: Follow directions, wear gloves and/or goggles as directed, properly drape client.

7. Apply your professional training.

8. Proper Use of First Aid: Keep first aid kit available in dispensary.

9. Fire Safety: Post and review evacuation procedure during new student orientation. In the event of fire: a) Contact fire department (Keep number readily available. Give name and address of business, nature of fire (what is burning), and name of person reporting the fire.

b) Evacuate premises by following the planned procedure for the facility.

c) Plan alternate exits for use in the event regular route is blocked by the fire.

d) Service extinguishers at last annually. Use of extinguishers. Install away from potential fire hazards and near an escape route.

Follow the instructions.

Many work as follows: a. Pull the pin b. Aim the nozzle c. Squeeze the handle d. Sweep from side to side at base until fire goes out.

Recommended Procedures: The National Fire Protection Association recommends that you should ONLY stand and fight a fire if ALL the following are TRUE: a. Everyone is leaving the premises and fire department has been called. b. The fire is small and confined to the work area where it started (wastebasket, cushion, small appliance, etc.) c. You can fight the fire with your back to an escape route. d. Your extinguisher is rated for the type of fire you are fighting and is in good working order. e. You know how to operate the extinguisher.

PRIVACY POLICIES

Effective January 1, 2013, the Standards and Criteria require institutions to guarantee each student or parent or guardian if the student is a dependent minor access to the student’s records. The institution must require written consent from the student or guardian before releasing any student information in response to a third party request, other than a request by NACCAS, unless otherwise required by law.

Students and parent/guardians of dependent minors are guaranteed the right to access and review the student’s educational file. Students must submit a written request to review their file to the school director. The student will be granted supervised access to their records within five business days of the request. Any third party request for information will require written authorization from the student or parent/guardian of a dependent minor. Essentials Academy of Cosmetology requires a release form be completed for each third party request of information. Essentials Academy of Cosmetology provides access to student records without written consent to its accrediting agency, the United States Department of Education, the State Licensing Agency, and any other school official. The institution maintains a record of all release forms and requests for information.

Student services

JOB PLACEMENT POLICY

While the school cannot guarantee employment for graduates, assistance in finding suitable employment is provided by posting area job openings on a career opportunities bulletin board for students to review.

Students also receive training in professionalism and job search skills including how to write a resume, complete an employment application and prepare for an effective interview. The curriculum places a great deal of emphasis on how to obtain and retain employment after graduation. Graduates are encouraged to maintain contact with the school and follow-up with the school on current employment or employment needs. In addition, the school maintains a network of relationships with professionals and employers who provide mentoring to students while they are in school. Job referrals are made known to interested graduates as available.

STUDENT GRIEVANCE PROCEDURE

In accordance with the institution's mission statement, the school will make every attempt to resolve any student complaint that is not frivolous or without merit. Complaint procedures will be included in new student orientation thereby assuring that all students know the steps to follow should they desire to register a complaint at any time. Evidence of final resolution of all complaints will be retained in school

1. The student should register the complaint in writing on the designated form provided by the institution within 60 days of the date that the act which is the subject of the grievance occurred.

2. The complaint form will be given to the school Director.

3. The complaint will be reviewed by management and a response will be sent in writing to the student within 30 days of receiving the complaint. The initial response may not provide for final resolution of the problem but will notify the student of continued investigation and/or actions being taken regarding the complaint.

4. If the complaint is of such nature that it cannot be resolved by the management, it will be referred to an appropriate agency if applicable.

5. Depending on the extent and nature of the complaint, interviews with appropriate staff and other students may be necessary to reach a final resolution of the complaint.

6. In cases of extreme conflict, it may be necessary to conduct an informal hearing regarding the complaint. If necessary, management will appoint a hearing committee consisting of one member selected by the school who has had no involvement in the dispute and who may also be a corporate officer, another member who may not be related to the student filing the complaint or another student in the school, and another member who may not be employed by the school or related to the school owners. The hearing will occur within 90 days of committee appointment. The hearing will be informal with the student presenting his/her case followed by the school's response. The hearing committee will be allowed to ask questions of all involved parties. Within 15 days of the hearing, the committee will prepare a report summarizing each witness' testimony and a recommended resolution for the dispute.

School management shall consider the report and either accept, reject, or modify the recommendations of the committee. Corporate management shall consider the report and either accept, reject, or modify the recommendations of the committee.

7. Students must exhaust the institution’s internal complaint process before submitting the complaint to the school’s accrediting agency, if applicable.

STUDENT RECORDS

All personal records and files are kept in the administration office. All records are kept confidential, and under lock and key, accessible only to the administrative staff. All other persons needing access to these records or files must have a written release or a waiver previously signed by the student. Students shall have access to their files at the time, upon written request and under direct supervision of a school official.

A private office is available for student counseling where they are free to discuss any problems which might affect their schools progress or future employment. A list of local professionals such as doctors, lawyers, counselors, automotive repair, is kept for student referencing.

All instructors are expected to attend monthly meetings during which the following topics, as applicable will be discussed: Education, teaching schedules, student evaluations.

Student counseling activities.

Student recruitment and admissions.

New student orientations.

Job readiness training.

Student salon activities.

Licensure exams and results.

Student retention (withdrawals and graduates).

Student/graduate feedback.

Staff image and professional development.

Staff meetings will be held more frequently if needed. Assessment of feedback from students, graduates, and advisory committee members will be reviewed and any applicable improvements or changes will be documented.

INSTRUCTOR CONTINUING EDUCATION

All instructors are required by the school to complete a minimum of 12 contact hours per year, (based on employment anniversary) of continuing education training, at least four of which must be devoted to academic and practical learning delivery methods. It is the personal responsibility of each instructor to meet this requirement and provide documentation of such training to management for his/her personnel file. The school may periodically require employees to participate in job training or career development activities in addition to the regular work schedule. It is the goal of the school to actively participate in the development of its employees; however, it is believed that the employees must be willing to invest both time and, if necessary, money poured self-improvement and professional development.
