We offer individual or combination of Cosmetology, instructor, and Manicuring, training. The school is owned by Melissa Gibbens. Melissa has over twenty-five (25) years of Cosmetology and Manicuring experience as a salon owner, as well as a school owner. The school’s main emphasis is on modern techniques which include but are not limited to: haircutting, coloring, specialty coloring, corrective coloring, permanent waving, styling thermal curling iron, acrylics, and nail enhancement, facials, and waxing. You are trained at your own pace, one-on-one adult education. She maintains inventory, orders textbooks, student kits, and working supplies. She conducts theory and practical classes. She assumes responsibility for recruiting, enrollment, re-enrollment, staffing, counseling and guidance to both staff and students. Background: Missouri Cosmetologist/Manicurist/Instructor. Graduated from Missouri College of Cosmetology in 1984. Matrix Color Specialist. Melissa brings a vast knowledge of business administration, sales, and marketing as a Missouri Real Estate Broker and business owner. Ashely Moore-Director of Education/Instructor Ashley is under the direct supervision of the director (s), and assumes the responsibility for education and counseling. She oversees curriculum education activities and class schedules. Ashley conducts academic classes, educates and prepares students in all areas to ensure a passing ration of the highest percent. She assists with inventory, and maintains student academic records and progress reports. Background: Ashely is in charge while the owner is off the premises. Ashley has been personally trained in the salon by the owner\executive director. Ashley is a perfect addition to teaching the innovative methods that Essentials academy of cosmetology uses. Ashley has owned and operated her own salon; and now enjoys sharing her practical work and business knowledge with our students. Aubree Davidson-Instructor Background: Aubree is a graduate of Essentials Academy of Cosmetology. She currently works as a licensed Cosmetologist/Nail tech part time in an area salon which keeps her current in the latest trends, as well as instructing at Essentials Academy. Mrs. Rowe brings over 50 years of experience in management and book keeping. She has successfully managed many physicians’ offices, and consulted and done book keeping for numerous small businesses. We ask that students reserve the front spaces for handi-cap parking and clients. Please do not leave your vehicle after 10:00 PM without permission any vehicles left are subject to being towed. GENERAL ADMISSIONS/TUITION TRANSFER STUDENTS Any applicant who has prior training in cosmetology, or Manicuring and wishes to complete training in this school, must first fulfill the admissions requirements. Essentials Academy of Cosmetology has no maximum number of clock hours they will except as a transfer student. Students who transfer within the state must submit a valid transcript of hours prior to beginning school, or hours must be approved by the Missouri State Board of Cosmetology. Transcripts will not be accepted after you have enrolled unless noted in the contract. The students will be evaluated and placed in the proper category, theory hours that are not part of a valid transcript may not be accepted, and a written test will be given to determine theory progress. This school does not recruit students who are currently registered, enrolled, or attending another school. TRANSCRIPT POLICY Essentials academy of cosmetology may accept clock hours from another institution once the following conditions are met: o o A transcript accepted by the Missouri State board of cosmetology showing the number of accumulated hours that are transferable for the course. The school may require practical and written evaluation to determine hours students will be given. Students exceeding their contract date will be charged a fee of up to $250 for each week over contract until completion of program. The program is 1500 clock hours; approximately 11 months at 34 hours per week, or part-time approximately 17 months at 20 hours a week. Plus a passing grade of 75% on the exams. Near completion of clock hours, each student will be given a mock board. A minimum of two instructors will administer the practical exam. The practical exam will be given in the exact format that is required for testing by the Missouri State board of cosmetology. There will also be 125 question written exam that will comprise of all areas of cosmetology and infection control. There will also be 125 question written exam that will be comprised of all areas of manicuring and infection control. Both portions of the exam must be passed with a minimum score of 75%. In order to take the Instructor course, the applicant must hold a current license as a Cosmetologist, Manicurist or Esthetician in the state of Missouri, or a state that accepts reciprocity with the state of Missouri. The program is approximately 5 months long at 32 hours per week, or part-time approximately 7 months at 20 hours per week. Near completion of clock hours, each student will be given a mock board. A minimum of two instructors will administer the practical exam. The practical exam will be given in the exact format that is required for testing by the Missouri State Board of Cosmetology. There will also be a 125 question written exam that will be comprised of all areas of cosmetology and infection control, and teaching methodology. STUDENT GUIDLINESS PERSONAL CONDUCT AND APPEARANCE All students will be required to maintain a professional attitude and professional conduct at all times. Misuse of policy rules and regulations may be grounds for termination. Foul language will not be tolerated. All services provided, whether to a student or public, must go through the front desk prior to the start. Black or white slacks, skirts, shorts (as long as they are about knee length) with a white or black shirt or an Essentials academy of cosmetology t-shirt, is considered proper attire. Please make sure your shoulders are covered, feet are covered, and your clothing is clean. Jeans may be worn on Saturdays only. It is encouraged to maintain a professional appearance at all times as we have many salon owners that stop by the academy to meet our students. Aprons or smocks need to be worn when on the student salon floor. It is your responsibility make sure they are kept clean. Hair, nails, and make-up should be done before you arrive at school! Students not in compliance with dress code will be asked to clock out and go home to change. LUNCH AND BREAKS It is the student’s responsibility to make sure a lunch is taken any day they are in attendance for five or more hours. A (1) one hour lunch break should be taken during the day. When providing services on the student salon floor it is the student’s responsibility to notify an instructor that they need to take a lunch break. Please try to notify instructor before beginning a service, if at all possible, if you have not had a lunch break. VISOTRS/CELL PHONES/FAMILY SERVICES/MISC. Please keep personal visits to a minimum. Phone calls on the school phone should be kept short as this is a business line. If you bring your cell phone to school, it must stay in the student lounge not on the student salon floor and in the classroom. We encourage you to bring your families in for salon services; however, we expect them to pay the usual prices. If it is someone you are financially responsible for, then there is only a charge for the product used. Items left on school premises more than 30 days will be considered as stored, and storage fees will apply. Items left more than six months will be considered abandoned and become the property of the school. 2. The school is not responsible for lost or stolen items while on school premises. Students are advised to label their items. CLASSES AND SCHEDULE You set your schedule at the time of contract signing. Schedules can include no less than 20 hours per week unless specially approved by the executive director. Your theory class time and Saturdays. If an exemption is obtained, projects, computer tutors, and library hours are considered theory hours Students may be taught under the supervision of instructor or instructor student during the hours the student is on premises. Students clocking in and doing nothing will not be given the time. You must be actively busy with coursework from the course you have enrolled in for hours to be credited. A Special lab class may be conducted any day by a guest artist, or instructor you will need to be flexible in your schedule availability. All training classes will be in English unless otherwise specified. Instructor boards are in English only. When creating your schedule, determine your concentration is your career choice. Students shall meet the minimum required units, regardless of hours, for eligibility to receive a certificate of completion. A grade of 75% will be earned only after requirements of both the state of Missouri and Essentials Academy of Cosmetology are fulfilled. Required hours are to be completed on school premises. Time will not be given for work outside of the school and is illegal without proper licensure or permission in advance to the state of Missouri. Students will follow all state laws and regulations. State law strictly forbids the practice of cosmetology/esthetics/Manicuring without a license. Students must be under the supervision of a licensee instructor. Violators will be reported to the Missouri State board of cosmetology. SATISFACTORY ACADEMIC PROGRESS ACADEMIC PROGRESS EVALUATIONS The Satisfactory Academic Progress Policy is consistently applied to all students enrolled at the school. It is printed in the catalog to ensure that all students receive a copy prior to enrollment. EVALUATION PERIODS Students are evaluated for Satisfactory Academic Progress as follows: Cosmetology 450, 900, and 1350 clocked (actual) hours Manicuring 200, 400 clocked (actual) hours Instructor 300, 600 clocked (actual) hours *Transfer Students- Midpoint of the contracted hours or the established evaluation periods, whichever comes first. Evaluations will determine if the student has met the minimum requirements for satisfactory academic progress. The frequency of evaluations ensures that students have had at least one evaluation by midpoint in the course. ATTENDANCE PROGRESS EVALUATIONS Students are required to attend a minimum of 67% of the hours possible based on the applicable attendance schedule in order to be considered maintaining satisfactory attendance progress. Evaluations are conducted at the end of each evaluation period to determine if the student has met the minimum requirements. The attendance percentage is determined by dividing the total hours accrued by the total number of hours scheduled. At the end of each evaluation period, the school will determine if the student has maintained at least 67% cumulative attendance since the beginning of the course which indicates that, given the same attendance rate, the student will graduate within the maximum time frame allowed. ACADEMIC PROGRESS EVALUATIONS The qualitative element used to determine academic progress is a reasonable system of grades as determined by assigned academic learning. Students are assigned academic learning and a minimum number of practical experiences. Academic learning is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion only when rated as satisfactory or better. If the performance does not meet satisfactory requirements, it is not counted and the performance must be repeated. Practical skills are evaluated according to text procedures and set forth in practical skills evaluation criteria adopted by the school. Students must maintain a written grade average of 75% and pass a FINAL written and practical exam prior to graduation. Students must make up failed or missed tests and incomplete assignments. Numerical grades are considered according to the following scale: 93 - 100 85 - 92 75 - 84 74 and BELOW EXCELLENT VERY GOOD SATISFACTORY UNSATISFACTORY DETERMINATION OF PROGRESS STATUS Students meeting the minimum requirements for academics and attendance at the evaluation point are considered to be making satisfactory academic progress until the next scheduled evaluation. Students will receive a hard-copy of their Satisfactory Academic Progress Determination at the time of each of the evaluations. Students deemed not maintaining Satisfactory Academic Progress may have their Title IV Funding interrupted, unless the student is on warning or has prevailed upon appeal resulting in a status of probation. WARNING Students who fail to meet minimum requirements for attendance or academic progress are placed on warning and considered to be making satisfactory academic progress while during the warning period. The student will be advised in writing on the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the warning period, the student has still not met both the attendance and academic requirements, he/she may be placed on probation and, if applicable, students may be deemed ineligible to receive Title IV funds. PROBATION Students who fail to meet minimum requirements for attendance or academic progress after the warning period will be placed on probation and considered to be making satisfactory academic progress while during the probationary period, if the student appeals the decision, and prevails upon appeal. Additionally, only students who have the ability to meet the Satisfactory Academic Progress Policy standards by the end of the evaluation period may be placed on probation. Students placed on an academic plan must be able to meet requirements set forth in the academic plan by the end of the next evaluation period. Students who are progressing according to their specific academic plan will be considered making Satisfactory Academic Progress. The student will be advised in writing of the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the probationary period, the student has still not met both the attendance and academic requirements required for satisfactory academic progress or by the academic plan, he/she will be determined as NOT making satisfactory academic progress and, if applicable, students will not be deemed eligible to receive Title IV funds. RE-ESTABLISHMENT OF SATISFACTORY ACADEMIC PROGRESS Students may re-establish satisfactory academic progress and Title IV aid, as applicable, by meeting minimum attendance and academic requirements by the end of the warning or probationary period. 14 | P a g e Essentials Academy of Cosmetology HANDBOOK 2015 4302 Howard Bush Drive Neosho, MO 64850 Phone: (417) 451-0355 INTERRUPTIONS, COURSE INCOMPLETES, WITHDRAWALS Students who withdraw prior to completion of the course and wish to re-enroll will return in the same satisfactory academic progress status as at the time of withdrawal. APPEAL PROCEDURE If a student is determined to not be making satisfactory academic progress, the student may appeal the determination within ten calendar days. Reasons for which students may appeal a negative progress determination include death of a relative, an injury or illness of the student, or any other allowable special or mitigating circumstance. The student must submit a written appeal to the school on the designated form describing why they failed to meet satisfactory academic progress standards, along with supporting documentation of the reasons why the determination should be reversed. This information should include what has changed about the student’s situation that will allow them to achieve Satisfactory Academic Progress by the next evaluation point. Appeal documents will be reviewed and a decision will be made and reported to the student within 30 calendar days. The appeal and decision documents will be retained in the student file. If the student prevails upon appeal, the satisfactory academic progress determination will be reversed and federal financial aid will be reinstated, if applicable. NONCREDIT, REMEDIAL COURSES, REPETITIONS Noncredit, remedial courses, and repetitions do not apply to this institution. Therefore, these items have no effect upon the school's satisfactory academic progress standards. TRANSFER HOURS With regard to Satisfactory Academic Progress, a student’s transfer hours will be counted as both attempted and earned hours for the purpose of determining when the allowable maximum time frame has been exhausted. EXTRA-INTRUCTIONAL CHARGES Each course/program has been scheduled for completion within an allotted time frame. A grace period of approximately 10% has been added to the calculated completion date for each program. It is not realistic to expect to receive an education for free. The school has reserved space, equipment, and licensed instructors for each student and course/program. If a student does not graduate within the contract period, additional training will be billed at the rate of $250 per week, or any part thereof, payable in advance, until graduation. Students will not be allowed to clock in until applicable weekly payments are made. COURSE REPETITION If a student needs to repeat a course, for any reason they may be subject to the current RE-ENTRY fee. If they are accepted and were making satisfactory progress as defined in the satisfactory academic progress policy section, at that point of withdrawal, they will be considered making satisfactory progress at the point of RE-ENTRY and will RE-ENTER in the same status as in place as time of departure. If the student did not meet the minimum requirements for satisfactory progress of the time of withdrawal, they may be placed on probation until the next scheduled evaluation point and all elements of Satisfactory Academic Policy are meet, if accepted for re-enrollment. 15 | P a g e Essentials Academy of Cosmetology HANDBOOK 2015 4302 Howard Bush Drive Neosho, MO 64850 Phone: (417) 451-0355 SALON FLOOR AND STUDENT SERVICES As a rule clients come first; however, here at essentials we understand that students desire to have student services. Students may receive student services when we have a slow time, or with the mission of the instructor, you may perform student services on other students or vice versa. If you are having a Chemical Service you must pay in advance for product you are using. In Booking Salon Floor services instructors will make their best effort in distributing clients fairly among students. If a client request a specific student then that request will be honored. SUBSTITUTE INSTRUCTORS A list of qualified substitutes will be available. All substitute instructors will attend annual staff meetings, and an orientation, complete routine employment paperwork, and provide the following documents: a current Missouri instructor’s license and documentation of current continuing education classes. The owner, Melissa Gibbens, is a qualified instructor in both cosmetology and Manicuring. She will be readily available in most cases that a substitute is needed. CERTIFICATE OF COMPETION Upon satisfactory completion of the required hours, all fees and charges are paid, and all paperwork is completed, a certificate of completion is issued stating that the student has completed adult education as prescribed by the Missouri State Board of Cosmetology. We do not guarantee the successful completion or the passing, of the Missouri State Board of Cosmetology examinations. Students must put forth the effort Attendance policy ATTENDANCE Taking time off could cause you to go beyond your enrollment agreement contract date, resulting in charges per that agreement. A doctor’s excuse is required if you are absent for medical reasons. Students will be given a 30 day period to make up missed or failed assignments or tests. If assignments or exams are missed due to unexcused absence, the student will make up assignments when the material is covered in the rotation of the curriculum, as all subject matter is usually covered again during the course length. Students absent for more than 14 days will be terminated according to school policy unless otherwise excused. PERSONAL LEAVE OF ABSENCE Essentials Academy of Cosmetology does not offer leave of absences for any reason. 16 | P a g e Essentials Academy of Cosmetology HANDBOOK 2015 4302 Howard Bush Drive Neosho, MO 64850 Phone: (417) 451-0355 Withdrawal/Termination Policy WITHDRAWL FROM INCOMPLETE COURSE Any student who withdraws from their contract and course, or fails to complete their training, will have a notice placed in their student file as to progress at the point of withdrawal. They will become subject to all fees as prescribed in the refund policy/cancellation settlement section. TERMINATION Below is a list of some of the most common reasons for termination: The use of alcohol and/or drugs on school premises. Padding of operational sheets, i.e.: Daily and or monthly worksheets Insubordination towards clients, fellow students, instructors or any staff member. Disruption of class. Obtaining credits by dishonest means. Failure to maintain a satisfactory attendance. Breach of contract. Not keeping up with assignments. Missing scheduled monthly payments. Practicing cosmetology away from the campus without being under the supervision of an instructor and permission from the Missouri State board of cosmetology as a field trip. RE-ENTRY/ RE-ENROLLMENT AFTER TERMINATION After being terminated for any reason, a student may be re-admitted at the same satisfactory progress status as the time of termination or withdrawal if the following conditions are met: The student must satisfy all charges incurred prior to termination, including charges computed and on policy/cancellation settlement section The student will be subject to the current RE-ENTRY fee. The student will re-register, and sign a new enrollment agreement with re-enrollment date. This means that he/she must pay current cost of tuition and fees and must also pay the registration fee again; however, the previous hours earned shall be used as credit in computing new charges. The student agrees to be placed on probationary status for 30 days or until the next evaluation period, after which the student’s grades, attitude, attendance, and general progress will be reviewed and evaluated in order to be considered for ongoing acceptance. If the student has not satisfactorily met the requirements for acceptance, termination will become final. 17 | P a g e Essentials Academy of Cosmetology HANDBOOK 2015 4302 Howard Bush Drive Neosho, MO 64850 Phone: (417) 451-0355 SPECIAL EXEMPTIONS WAIVERS If for any reason a student is unable to meet their contractual obligations they may request a special exemption waiver. This waiver allows the director to make exceptions to written policy, based on extenuating circumstances. Failure to obtain a special exemption waiver may result in actions being taken against the student. EXTENUATING CIRCUMSTANCES Situations sometimes arise where a student is unable to abide their contractual obligations. Some of those situations will be due to extenuating circumstances. In dealing with these situations, such circumstances may be taken into consideration through the use of a special exemption waiver. Below are some examples of extenuating circumstance, which may cause the student to be unable to meet their contractual obligations: Hospitalization of students or serious injury or illness. Doctor’s orders to remain in effect for a specific number of days beyond five. Medical release to return to school is required Circumstances occurring beyond the student’s control. Child care issues Employment obligations REFUND/CANCELLATION POLICY For applicants who cancel enrollment or students who withdraw from enrollment a fair and equitable settlement will apply. The following policy will apply to all terminations for any reason, by either party, including student decision, course or program cancellation, or school closure. Any monies due the applicant or students shall be refunded within 45 days of official cancellation or withdrawal. Official cancellation or withdrawal shall occur on the earlier of the dates that: 1. 2. 3. 4. 5. 6. An applicant is not accepted by the school. The applicant shall be entitled to a refund of all monies paid. A student (or in the case of a student under legal age, his/her parent or guardian) cancels his/her enrollment in writing within three business days of signing the enrollment agreement. In this case all monies collected by the school shall be refunded, regardless of whether or not the student has actually started classes. A student cancels his/her enrollment after three business days of signing the contract but prior to starting classes. In these cases he/she shall be entitled to a refund of all monies paid to the school less the registration fee in the amount of $100. A student notifies the institution of his/her withdrawal in writing. A student is expelled by the school. (Unofficial withdrawals will be determined by the institution by monitoring attendance at least every 30 days.) In type 2, 3, 4 or 5, official cancellations or withdrawals, the cancellation date will be determined by the postmark on the written notification, or the date said notification is delivered to the school administrator or owner in person. For students who enroll and begin classes but withdraw prior to course completion (after three business days of signing the contract), the following schedule of tuition earned by the school applies. Any monies due a student who withdraws shall be refunded within 45 days of a determination that a student has withdrawn, whether officially or unofficially. In the case of disabling illness or injury, death in the student's immediate family or other documented mitigating circumstances, a reasonable and fair refund settlement will be made. If permanently closed or no longer offering instruction after a student has enrolled, the school will provide a pro rata refund of tuition to the student OR provide course completion through a pre-arranged teach out agreement with another institution. If the course is canceled subsequent to a student's enrollment, the school will either provide a full refund of all monies paid or completion of the course at a later time. Students who withdraw or terminate prior to course completion are charged a cancellation or administrative fee of $150.00. This refund policy applies to tuition and fees charged in the enrollment agreement. Other miscellaneous charges the student may have incurred at the institution (EG: extra kit materials, books, products, unreturned school property, etc.) will be calculated separately at the time of withdrawal. All fees are identified in the catalog and in this enrollment agreement. UNOFFICIAL WITHDRAWL Students who have 14 days of consecutive absence without contacting the school will be considered withdrawn. Clock hours will be checked on a minimum of every 30 days. If a student has been officially withdrawn refund policy will apply. CAMPUS CRIME AND SECURITY Essentials Academy of Cosmetology encourages all students and employees to be responsible for their own security and the security of others. Please report any known criminal offenses occurring on campus to the school administration. In the event a sex offense should occur on campus, the victim should take the following steps: 1. Report the offense to the school administration. 2. Preserve any evidence as may be necessary to the proof of the criminal offense. 3. Request assistance, if desired, from school administration in reporting the crime to local law enforcement agencies. 4. Request a change in the academic situation if necessary. Information for Crime Victim about disciplinary hearings: Institution must, upon written request disclose to the alleged victim of any crime of violence, or a no forcible sex offense, the results of any disciplinary proceeding conducted by the institution against a student who is the alleged perpetrator of 19 | P a g e Essentials Academy of Cosmetology HANDBOOK 2015 4302 Howard Bush Drive Neosho, MO 64850 Phone: (417) 451-0355 such crime or offense. If the alleged victim is deceased as a result of the crime or offense, the information shall be provided, upon request to the next of kin of the alleged victim. This provision applies to any disciplinary proceeding conducted by the institution on or after August 14, 2009 (HEOA amendment). Emergency Evacuation Plan This School’s campus consists of one building. If an emergency evacuation is required you will be notified by your instructor and follow the emergency plan posted in the area you are currently located in. On an annual basis, the institution conducts an emergency evacuation drill. Each classroom has emergency evacuation procedures posted in the room. Please make yourself familiar with these evacuation procedures. Drug and alcohol abuse information: Following you will find the requirements of the Drug-Free Schools and Communities Act Amendments of 1989, Public Law 101-226 and what Essentials Academy of Cosmetology requires of Staff and Students. Staff and Students are prohibited from the unlawful manufacture, distribution, possession, or use of illicit drugs or alcohol. This prohibition applies while on the property of the school or participating in any institutional activity, Students or employees who violate this policy will be subject to disciplinary action up to and including expulsion or termination from school or employment. The school will impose disciplinary actions consistent with local, State and Federal law. The actions will be determined by the school director after consultation with the U.S. Department of Education, local law enforcement officials, rehabilitation staff and others depending on the particular circumstances. Continuation as a student or as an employee at Essentials Academy of Cosmetology will depend on factors that include but are not limited to: the severity of the offense; completion of an appropriate rehabilitation program; frequency of the violation; arrest records and convictions *FOR ALL EMERGANCIES PLEASE DIAL 911* PRIVACY POLICIES Effective January 1, 2013, the Standards and Criteria require institutions to guarantee each student or parent or guardian if the student is a dependent minor access to the student’s records. The institution must require written consent from the student or guardian each time before releasing any student information in response to a third party request, other than a request by NACCAS, unless otherwise required by law. Students and parent/guardians of dependent minors are guaranteed the right to access and review the student’s educational file. Students must submit a written request to review their file to the school director. The student will be granted supervised access to their records within five business days of the request. Any third party request for information will require written authorization from the student or parent/guardian of a dependent minor. Essentials Academy of Cosmetology provides access to student records without written consent to its accrediting agency, the United States Department of Education, the State Licensing Agency, and any other school official. The institution maintains a record of all release forms and requests for information. Student services JOB PLACEMENT POLICY While the school cannot guarantee employment for graduates, assistance in finding suitable employment is provided by posting area job openings on a career opportunities bulletin board for students to review. Students also receive training in professionalism and job search skills including how to write a resume, complete an employment application and prepare for an effective interview. The curriculum places a great deal of emphasis on how to obtain and retain employment after graduation. Graduates are encouraged to maintain contact with the school and follow-up with the school on current employment or employment needs. In addition, the school maintains a network of relationships with professionals and employers who provide mentoring to students while they are in school. Job referrals are made known to interested graduates as available. STUDENT GRIEVANCE PROCEDURE In accordance with the institution's mission statement, the school will make every attempt to resolve any student complaint that is not frivolous or without merit. Complaint procedures will be included in new student orientation thereby assuring that all students know the steps to follow should they desire to register a complaint at any time. Evidence of final resolution of all complaints will be retained in school files in order to determine the frequency, nature, and patterns of complaints for the institution. The following procedure outlines the specific steps of the complaint process. 1. The student should register the complaint in writing on the designated form provided by the institution within 60 days of the date that the act which is the subject of the grievance occurred. 2. The complaint form will be given to the school Director. 3. The complaint will be reviewed by management and a response will be sent in writing to the student within 30 days of receiving the complaint. The initial response may not provide for final resolution of the problem but will notify the student of continued investigation and/or actions being taken regarding the complaint. 4. If the complaint is of such nature that it cannot be resolved by the management, it will be referred to an appropriate agency if applicable. 5. Depending on the extent and nature of the complaint, interviews with appropriate staff and other students may be necessary to reach a final resolution of the complaint. 6. In cases of extreme conflict, it may be necessary to conduct an informal hearing regarding the complaint. If necessary, management will appoint a hearing committee consisting of one member 21 | P a g e Essentials Academy of Cosmetology HANDBOOK 2015 4302 Howard Bush Drive Neosho, MO 64850 Phone: (417) 451-0355 selected by the school who has had no involvement in the dispute and who may also be a corporate officer, another member who may not be related to the student filing the complaint or another student in the school, and another member who may not be employed by the school or related to the school owners. The hearing will occur within 90 days of committee appointment. The hearing will be informal with the student presenting his/her case followed by the school's response. The hearing committee will be allowed to ask questions of all involved parties. Within 15 days of the hearing, the committee will prepare a report summarizing each witness' testimony and a recommended resolution for the dispute. School management shall consider the report and either accept, reject, or modify the recommendations of the committee. Corporate management shall consider the report and either accept, reject, or modify the recommendations of the committee. 7. Students must exhaust the institution’s internal complaint process before submitting the complaint to the school’s accrediting agency, if applicable. STUDENT RECORDS All personal records and files are kept in the administration office. All records are kept confidential, and under lock and key, accessible only to the administrative staff. All other persons needing access to these records or files must have a written release or a waiver previously signed by the student. Students shall have access to their files at the time, upon written request and under direct supervision of a school official. COUNSELING SERVICES A private office is available for student counseling where they are free to discuss any problems which might affect their schools progress or future employment. A list of local professionals such as doctors, lawyers, counselors, automotive repair, is kept for student referencing. Instructor guidelines STAFF MEETINGS All instructors are expected to attend monthly meetings during which the following topics, as applicable will be discussed: Education, teaching schedules, student evaluations. Student counseling activities. Student recruitment and admissions. New student orientations. Job readiness training. Student salon activities. Licensure exams and results. Student retention (withdrawals and graduates). Student/graduate feedback. Assessment of feedback from students, graduates, and advisory committee members will be reviewed and any applicable improvements or changes will be documented. INSTRUCTOR CONTINUING EDUCATION All of Essentials instructors are required to meet the Missouri State board of cosmetology in continuing education (C.E.) Standards, but not less than 12-clock hours of such activity each year, per instructor. Methodology. Each instructor reviews a written performance evaluation at least once every 12 months. The evaluation includes, at a minimum, teaching preparation and effectiveness. It is the personal responsibility of each instructor to meet this requirement and provide documentation of such training to management for his/her personnel file. The school may periodically require employees to participate in job training or career development activities in addition to the regular work schedule. It is the goal of the school to actively participate in the development of its employees; however, it is believed that the employees must be willing to invest both time and, if necessary, money poured self-improvement and professional development. Sources for continuing education: Community colleges or universities. Association for career and technical education. Cosmetology educators of America. Product manufacturers. In-house continuing education provided by qualified individuals. NACCAS workshops and seminars. Publishers in the applicable fields of study. Approved providers of continuing education recognized industry experts/trainers. Federal "Return Of Title IV Aid" Policy This policy applies to students who withdraw (officially, unofficially, or fail to return from a Leave of Absence) or are dismissed from enrollment at Essentials Academy of Cosmetology. It is separate and distinct from the Essentials Academy of Cosmetology Refund Policy. The calculated amount of the "Return of Title IV Funds" that is required for students affected by this policy are determined according to the following definitions and procedures, as prescribed by regulation: Essentials Academy of Cosmetology has 45 days from the date the institution determines that the student withdrew to return all unearned funds for which it is responsible. The academy is required to notify the student if he or she owes a repayment via written notice. The school must advise the student or parent that he or she has 14 calendar days from the date the academy sent the notification to accept a post-withdrawal disbursement. If a response is not received from the student or parent within the permitted time frame or the student declines the funds, the academy will return any earned funds that the academy is holding to the Title IV programs. Post-withdrawal disbursement must occur within 90 days of the date the 23 | P a g e Essentials Academy of Cosmetology HANDBOOK 2015 4302 Howard Bush Drive Neosho, MO 64850 Phone: (417) 451-0355 student withdrew. The Return of Title IV Funds (R2T4) regulation does not dictate the institutional refund policy. The calculation of Title IV funds earned by the student has no relationship to the student’s incurred institutional charges. A school is required to determine the earned and unearned Title IV aid a student has earned as of the date the student ceased attendance based on the amount of time the student spent in attendance. Withdrawal before 60% Essentials Academy of Cosmetology must perform an R2T4 to determine the amount of earned aid up through the 60% point in each payment period. The school will use the Department of Education’s prorated schedule to determine the amount of R2T4 funds the student has earned at the time of withdrawal. After the 60% point in the payment period, a student has earned 100% of the Title IV funds he or she was scheduled to receive during the period. The institution must still perform a R2T4 to determine the amount of aid that the student has earned. Withdrawal after 60% For a student who withdraws after the 60% point-in-time, there are no unearned funds. However, Essentials Academy of Cosmetology must still determine whether the student is eligible for a post-withdrawal disbursement. Withdrawals - Official vs. Unofficial A student who withdraws is one who either officially goes through a withdrawal from Essentials Academy of Cosmetology, unofficially withdraws (is administratively withdrawn), is suspended, or officially withdraws and submits it in writing to the school. A student's withdrawal date is determined by using one of the following: • the date the student began the institution's official withdrawal process or officially notified the institution of intent to withdraw; or • the midpoint of the period for a student who leaves without notifying the institution; or • the student's last date of attendance at a documented academic-related activity. Failure to Return from an Official Leave of Absence (LOA) If a student does not return on his or her scheduled date from a documented Leave of Absence, Essentials Academy of Cosmetology will withdraw the student ten days after they were scheduled to return. Their last day of attendance will then be the last day they attended an academically-related activity, prior to the LOA. Earned Title IV Aid Title IV Aid is earned in a prorated manner on a per diem basis (calendar days) up to the 60% point in the trimester. Title IV Aid is viewed as 100% earned after that point in time. Phone: (417) 451-0355 Federal Unsubsidized Direct Loan Federal Subsidized Direct Loan Federal Direct PLUS Loan Federal Pell Grant Other Title IV Aid Other Aid and/or Scholarships Essentials Academy of Cosmetology’s responsibilities in regard to the return of Title IV funds follow: • providing students with the information given in this policy; • identifying students who are affected by this policy and completing the Return of Title IV Funds calculation for those students; • returning any Title IV funds that are due the Title IV programs. The student's responsibilities in regard to the return of Title IV funds include: • returning to the Title IV programs any funds that were disbursed to the student and which the student was determined to be ineligible for via the Return of Title IV Funds calculation; • notifying of a withdrawal via written form; • notifying of an intent to rescind a withdrawal notice via written form; and • notifying the School Director of a notification of withdrawal or rescission of intent to withdraw. Post Withdrawal If you did not receive all of the funds that you earned, you may be due a post-withdrawal disbursement. If the post-withdrawal disbursement includes loan funds, the institution must get your permission before it can disburse them. You may choose to decline some or all of the loan funds so that you do not incur additional debt. The institution may automatically use all or a portion of your post-withdrawal disbursement (including loan funds, if you accept them) for tuition and fees). For all other institutional charges, the institution needs your permission to use the post-withdrawal disbursement. If you do not give your permission, you will be offered the funds. However, it may be in your best interest to allow the school to keep the funds to reduce your debt at the school. If the institution is not required to return all of the excess funds, you must return the remaining amount. Any loan funds that you must return, you (or your parent for a PLUS Loan) repay in accordance with the terms of the promissory note. That is, you make scheduled payments to the holder of the loan over a period of time. Any amount of unearned grant funds that you must return is called an overpayment. The amount of a grant overpayment that you must repay is half of the grant funds you received or were scheduled to receive. You must make arrangements with the institution or the Department of Education to return the unearned grant funds. The requirements for Title IV program funds when you withdraw are separate from the Essentials Academy of Cosmetology Refund Policy. Essentials Academy of Cosmetology may also charge you for any Title IV program funds that the academy was required to return. VERIFICATION POLICY The U.S. Department of Education (DOE) requires certain student applicants to verify the accuracy of information provided when applying for federal aid. Any student may be asked to verify application information at the professional discretion of the financial aid officer. Requirements are applicable to all applicants for federal student aid under the following programs: The Federal Pell Grant Program The Federal Stafford Direct Loan Program Federal Supplemental Educational Opportunity Grants (FSEOG) Federal Work-Study (FWS) Selection For Verification If a student's application is selected for review by the federal audit system, the Financial Aid Office is required to verify the application information of those students prior to disbursing federal student aid funds. No aid will be disbursed until completion of the verification process and proper submission of additional forms for loans, grants, or other federal aid. STUDENTS ARE RESPONSIIBLE FOR PAYMENT OF TUITION BY THE PAYMENT DEADLINE REGARDLESS OF THE STATUS OF THE VERIFICATION PROCESS. FINANCIAL AID WILL NOT DEFER TUITION PAYMENT IF THE STUDENT HAS OUTSTANDING REQUIREMENTS THAT ARE NOT SATISFIED BY AUGUST 1, PRIOR TO THE TUITION DEADLINE OF AUGUST 10. Students may be subject to a late fee or ultimately being withdrawn from classes for non-payment if financial aid is not finalized and ready to disburse by the tuition deadline. Payment can be made by check, credit card, or monthly payment plan to avoid penalty. If tuition has been paid in full at the time any financial aid for which the student is eligible is ready to disburse, it will be disbursed to the student and refunded in the form of a check. Discretionary Verification Individual institutions have the authority to verify any student's information and ask for any additional documentation needed to insure the integrity of the federal programs and to aid them in the smooth operation of their offices. If the student submits any discrepant information or the FAO has any reason to believe the applicant information is not correct, the student will be required to provide documentation to the extent necessary to resolve the conflict. All conflicts must be resolved before federal aid can be disbursed. Transfer students whose application was selected by the Central Processor for verification will be verified completely regardless of whether that student was verified at their previous institution. The verification completed by AMU and subsequent recalculation of EFC, if any, supersedes computation of EFC as a result of verification by another institution. 26 | P a g e Essentials Academy of Cosmetology HANDBOOK 2015 4302 Howard Bush Drive Neosho, MO 64850 Phone: (417) 451-0355 Unsubsidized Loans An applicant who has been selected for verification but is eligible only for Unsubsidized Federal Stafford Loans may be excused from completing the verification process; in other words, a student with an expected family contribution (EFC) that is too high to qualify him or her for a Subsidized Federal Stafford Loan would not have to complete verification. A student who might be eligible for a subsidized loan cannot waive verification by choosing to forfeit any need-based grants and borrow an unsubsidized loan only. Forms and Documentation Collection Procedures And Deadlines Essentials Academy of Cosmetology requires all applicants subject to verification to fill out a Federal Verification Worksheet and return it to the Financial Aid Office before any further processing will be done on their aid package. Applicants should return Federal Verification Worksheet, along with appropriate signed tax returns, prior to priority deadline of April 1st for consideration of federal and institutional need-based grants. Students can submit verification documents at any time up to the deadline.* Applicants selected for verification will not be packaged for financial aid until the process is completed. If verification is not completed, no Title IV or need-based aid will be awarded or disbursed. If verification is not complete by the payment deadline, students must pay tuition, fees, and room and board by other means. *A Pell applicant selected for verification must complete the process by the deadline published in the Federal Register. Verification is complete when an Essentials Academy of Cosmetology Financial Aid Officer has received and reviewed all the requested documentation. For Pell grant eligibility, the students' SAR must have been corrected or have shown that the information is correct, and the school must have his valid correct ISIR or SAR. For Campus-based and Stafford loan funds, you must have an ISIR or SAR with an official EFC that shows the application data were processed through the CPS at least once while the student was enrolled. Independent Students - must provide signed copies of tax return and Verification Worksheet, if student is married spouse must also provide a copy of his or her tax return. Dependent Students - must provide signed copies of their tax return and Verification Worksheet as well as provide signed copies of their parents' tax return. Definition of Parent is not limited to natural parents. The following are the guidelines for establishing parental status: Adoptive parent: Treated as natural parent. Foster Parent: Not treated as a natural parent unless directed by the court to use personal resources to support the student Grandparent or other relatives: not treated as a natural parent unless he or she has legally adopted the student. Stepparent: Treated as natural parent if they were married to the natural parent at the time the application was signed. If the natural parent dies and the stepparent survives the student becomes dependent upon the surviving natural parent, unless the stepparent legally adopted the student. Non-Filers: For students not required to file a tax return, a properly filled out Verification Worksheet should reflect any earnings, and should be accompanied by any W-2's the student may have. Household Size Number of persons in the household that the student (and their spouse) or dependent student's parent(s) will support during the award year. 2. Number of Household members Enrolled in Postsecondary Education at least half-time (six credit hours) for at least one term. 3. Adjusted Gross Income or income earned from work if AGI has not been calculated, either amount must be for the base year. 4. U.S. Income Tax Paid for base year. 5. Certain Untaxed Income and Benefits for the base year: a) Social Security b) Child Support c) Untaxed payments to IRA and/or Keogh plans d) Foreign Income Exclusion e) Earned Income Credit f) Interest on tax free bonds g) Other untaxed income from the U.S. income tax return (i) Tax Exempt Interest Income (ii) Untaxed Portions of Pensions NOTE: All tax returns must be signed by the applicant or the tax preparer Applicants will be sent notification after the verification and recalculation process has been completed and the student's completed financial aid package has been determined. VA ATTENDANCE POLICY ADDENDUM VA eligible students must maintain an attendance average of at least 80%, evaluated monthly. Failure to maintain this standard will lead to a 30 day probationary period. AVAILABILITY OF STUDENT RECORDS Students may review their school records upon request. PRIOR CREDIT VA eligible students with prior credit will have their previous coursework evaluated.
