RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING NYU Hospitals Center – Kimmel Program Building Commissioning Services NYU Hospitals Center Kimmel Pavilion Building Commissioning Services Request for Proposal and Process 1) Introduction The NYU Hospitals Center (the “Medical Center”) requests proposals from qualified firms to perform Commissioning Authority ("CxA") services for the Kimmel P avilion Building located at the existing East Midtown campus. The selected Commissioning Authority will coordinate the commissioning process, will help to develop commissioning schedules, will develop checklists, oversee testing, and will produce a Final Commissioning Report in accordance with the scope set forth in this document. We expect that the selected Commissioning Authority will oversee several Commissioning Agents, under their direct management & employment, who will perform the actual testing and calibrations, with cooperation and in the presence of the Subcontractors. The Commissioning Agents can be either direct employees of the Commissioning Authority or a subcontracted entity. We prefer to see a substantial percentage of personnel assigned to this project as direct employees of the Commissioning Authority. In the event that the Commissioning Agents will be subcontracted, this must be made transparent in your proposal. Resumes for each team member are a mandatory inclusion in your proposal. Teams will be specifically contracted by individual and any changes to the team either prior to start to the project or during the project must be submitted in writing and approved by the Owner in advance of such change in personnel taking place. The Commissioning Authority will notify Owner two weeks in advance of all commissioning activities and testing, which the Owner will have the right participate in and/or witness. The Kimmel Pavilion (KP), built as part of the Kimmel Program work, is roughly 800,000 square feet. Building services include chilled water, high/medium/low pressure steam, hot water, and related support services. N ormal and emergency electrical distribution will be provided from the new Energy Building (EB) being built concurrently with this project. The equipment located in the EB will be commissioned unde r a separate contract. Building services shall be commissioned from the energy transfer station in the building out through the end devices (secondary loop). Additional building services including central building vacuum and compressed air and gases shall also be included and commissioned as part of this contract. Main building HVAC systems and patient spaces including operating and procedure rooms shall be commissioned in their entirety. Page 1 of 27 RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING The Medical Center requires all projects be commissioned by a third party commissioning Authority to assure that the completed building meets all engineering design parameters and operates as engineered. This project will pursue LEED certification so the CxA will be required to follow all processes and fulfill the required documentation to ensure compliance with LEED. The Medical Center intends to achieve minimum LEED Silver and target LEED Gold, including the LEED point for enhanced commissioning (EAc3) and proposals should reflect this goal. The Commissioning Authority also is required to ensure that the patient rooms and all other patient occupied spaces meet Joint Commission (JCAHO) accreditation. 2) Scope of Commissioning and Start -Up Engineering Services The Complete Scope of Commissioning Services is defined as in ATTACHMENT A, Project Specific Scope of Services. Inadvertently Omitted Scope or Services Proposing firms are encouraged to bring to the attention of the Medical Center any scope or services it deems necessary and/or appropriate for the p roject but that may have been inadvertently omitted from the Request for Proposal. This information shall be reviewed by them and sent to all bidders as a bid clarification. 3) NYU Hospitals Center Consultants Agreement The enclosed Agreement “ATTACHMENT C” sets forth the services and conditions of the proposed engagement Requests for deviations from the Agreement must be clearly identified in the proposal and should not be assumed to be agreed upon . Clarifications Questions with respect to this RFP must be submitted in writing by March 28 t h , 2011. Answers to the questions will be distributed in writing to all Proposers by April 1 s t . 4) Proposal Content and Format a) General Adherence to the specified proposal format is mandatory . Brevity, conciseness, and responsiveness are paramount. For the purposes of the proposal the following major wor k phases can be assumed. Proposal shall be broken down to indicate the cost for each phase, as well as the anticipated man-hours to be used. The costs and man -hour requirements shall be located in the fee section of the proposal. The commissioning scope must fulfill, but will not be solely defined by the LEED Commissioning Prerequisite and credit for En hanced Commissioning. Proposers must review the Scope of Services ATTACHMENT A and fully commit to all requirements that may be in addition to or an enhancement of the LEED checklist. The proposal must include resumes of partner/principals and key sta ff and indication of their ability to commit to the Project. The proposal must include relevant technical information covering your work process and product. Including but not limited to: methodologies, applications, quality assurance processes, etc. The proposal must include description the proposed commissioning teams’ experience in the application of the latest version of the following ASHRAE Guidelines: 62 – Air Quality Standards 90.1 – Energy 55 – Thermal Environmental Conditions for Human Occupancy 62.1 - Ventilation for Acceptable Indoor Air Quality 0-2005 – The Commissioning Process 1-1996 – The HVAC Commissioning Process Provide specific examples. A matrix identifying tasks, labor hours, rates, and subtotal / totals shall be included as a part of the proposal. Staffing Plan Prepare a detailed staffing plan to support the Work Plan and Schedule. Show how Staffing Plan meets monthly manpower levels required by Schedule above. We will contractually obligate the selected firm to maintain the project team over the life of this assignment unless a proposed change in team member is requested in writing and pre approved by the NYU Hospitals Center staff in advance. Indicate monthly staffing levels to achieve schedule. We strongly prefer a minimum of five years experience for each team member. As a supplement to your staffing plan, please provide a Table of Organization in the form of a diagram illustrating the structure your project team as well as the relationships and relative ranks of the various positions. A Cost Matrix shall be used to detail fees for each phase of the project. Our format for the cost matrix is included as Attachment B. GENERAL: Project Specific Scope of Services General: Proposals shall be based on services that include the following: Review of the Owner’s Project Requirements (OPR) and Basis of Design (BOD) 1 Participation in the design review (for commissionability and operability of designed systems). Involvement during the construction phase Development of a commissioning program, and carrying out that program The observation of the start-up of all systems Post-commissioning performance verification of the completed systems in compliance with the design basis and equipment specifications prior to expiration of the equipment warranties These services are customarily performed by consultants required for projects of this type and scope. The Engineering Design Review will be involved during all phases of document development The Commissioning Authority (CxA) will develop an overall co mmissioning program and working with the Kimmel Program Director to oversee the activities of all other project team members to ensure compliance with the program. Such procedures will ensure that the start-up and commissioning processes comply with the Original Equipment Manufacturer's (OEM) standards and procedures, and ensure that all testing and commissioning work is successfully completed within the contract completion period. The CxA will be engaged by the Owner. The design Engineer of Record and Owners Faci lities Engineering and Commissioning groups will be the core of the project quality assurance program. Documentation will flow from the OEM representatives and Testing and Balancing Contractor to the Kimmel Program Director who will distribute to the CxA in order to allow the CxA to verify that start-up and commissioning tests and protocols were followed. The CxA will submit all reports and recommendations to the Kimmel Program Director for final approval and disposition. 1 This is required by the LEED Pre-Requisite, and the OPR will be prepared by the LEED consultant (Atelier Ten) in tandem with the Owner. Coordination of documents for the grant will be performed by others. In addition to the overall responsibilities outline d above, the selected firm will provide but not be limited to, the services listed below : 2. DESIGN PHASE: Review the Owner’s Project Requirements (OPR) and Basis of Design (BOD), as required by LEED. Weekly Project Meetings : CxA shall attend all scheduled project meetings for the duration of the design phase. CxA shall represent the NYU Hospitals Center Facilities Operations in these meetings and shall note any issues that impact the operations department discussed at the meetings . CxA shall summarize the relevant issues in writing and bring them to the attention of the NYU Hospitals Center Kimmel Program Director for response and disposition. CxA should assume weekly meetings with the project team throughout design development. In addition to the weekly meetings 2 additional meetings for each design review milestone shall be included in the proposal. Review all MEP drawings and specifications related to the building and thermal power process systems. These reviews shall have two (2) occurrences during design development (50% and 100% DD), and two (2) occurrences for contract documents (50% and 100% CD). These reviews are for checking for completeness and adherence to the original design intent (design basis) as well as commissionability of systems. Review contract documents to assure complete coordination among the various disciplines, as well as how they relate to construction sequences, materials, storage, site and building access, and equipment maintenance access. Develop and maintain a Design Deficiency Report detailing issues generated through the review process from both the CxA and NYU Hospi tals Center Operations. Consolidate comments into a single report and submit to the Kimmel Program Director for incorporation into the Page 11 of 27 RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING design documents. Verify comments have been incorporated into the design or responded to by the Engineer of Record. Design Deficiency Report shall have at a minimum : the item or deficiency, a unique deficiency number, the location of the deficiency in sufficient detail to locate on design documents (include Specifica tion, Drawing Number, Column Lines, Paragraph Number, etc.) date added to report, agreed upon remediation or action from the Design Team, and date of Design Team action. Other information may be added as required or requested by the NYU Hospitals Center . Participate in Value Engineering exercises to ensure proje ct stays within the design intent. Provide feedback and recommendations for reducing project costs. Include results of Value Engineering items on Design Deficiency Report . Develop the Commissioning Plan and necessary functional performance tests, procedures and specifications that will be provided to the engineer of record for incorporation into the contract documents. The Commissioning Plan should describe the systems scheduled for commissioning, the nature of the testing to be performed, & those required to be in attendance. The documentation of the commissioning of systems and controls shall include normal operating modes and emergency operation. Outline test plans for each system and piece of equipment to be commissioned . Include manpower requirements for construction trades where applicable as well as witnessing / documenting all MEP/FA tests Produce a commissioning schedule to be coordinated and incorporated into the overall project schedule Include time for both equipment and systems testing (where applicable). The specifications shall be reviewed and a documentation submission list developed to assure that the Owner receives all necessary documenta tion from the Contractor, that clear procedures are established for the commissioning process, that any special equipment or instrumentation needed for obtaining measurements during performance testing is included, and that there is a well defined training program to assure that the building ’s operation personnel receive adequate training for the p roper operation of the bui lding equipment and systems. 3. PRE-CONSTRUCTION PHASE Project Meetings: CxA shall attend all scheduled project meeti ngs for the duration of the pre-construction phase. CxA shall represent NYU Hospitals Center Facilities Operations in these meetings and shall note any issues that impact the operations department discussed at the meetings . CxA shall summarize the relevant issues in Page 12 of 27 RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING writing and bring them to the attention of the Kimmel Program Director for response and disposition. CxA should assume monthly meetings with the project team throughout the pre-construction phase. 4. TRADES BIDDING PHASE Project Meetings: CxA shall attend all scheduled project meetings for the duration of the trades bidding phase. CxA shall represent NYU Hospitals Center Facilities Operations in these meetings and shall note any issues that impact the operations department discussed at the meetings. CxA shall summarize the relevant issues in writing and bring them to the attention o f the NYU Hospitals Center Kimmel Program Director for response and disposition. CxA should assume weekly meetings with the project team throughout the trades bidding phase. Chair a post bid-award meeting with MEP and specialty trades to define what is involved in commissioning, review of the seq uence and schedule of this work and an explanation of its importance to the project. Include anticipated manpower requirements and timeframes for the commissioning work . Develop a comprehensive list of deliverable s broken out by trade inclusive of spare parts, training, reports, documentation, and manuals. This list will continue to be updated to track progress and inform the operations & maintenance group. Prior to bid award review the MEP trades proposed scope of work to ensure all work, including commissioning time has been captured in the bids . Provide any clarifications to the NYU Hospitals Center Kimmel Program Director for inclusion in the trades bids. 5. CONSTRUCTION PHASE Project Meetings: CxA shall attend all scheduled project meetings for the duration of the pre-construction phase CxA shall represent NYU Hospitals Center Facilities Operations in these meetings and shall note any issues that impact the operations department discussed at the meetings. CxA shall summarize the relevant issues in writing and bring them to the attention of the NYU Hospitals Center Kimmel Program Director for response and disposition. CxA should assume weekly meetings with the project team throughout the construction phase. Develop and maintain an internet based construction deficiency report which will be submitted to the NYU Hospitals Center Kimmel Program Director for correction. During project meetings, discuss issues and coordinate with NYU Hospitals Center Kimmel Program Director to ensure corrections & rework are scheduled and completed by trades. Verify corrections have been made as required . Page 13 of 27 RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING Construction deficiency report shall have at a minimum the item or deficiency, a unique deficiency number, the location of the deficiency in sufficient detail to locate upon construction completion (include room numbers, floor, North /East/West/ South. etc.), date added to report, agreed upon remediation or action from the Contractor, date remediation completed and any photographs of the item or deficiency. Other information may be added as required or requested by the NYU Hospitals Center team. Conduct periodic commissioning team meetings. It is recommended as one meeting every other week during early stages of construction and transitioning to a weekly meeting upon start of the testing phase through completion. Note that these meetings may be combined with the project meetings. If this is assumed, indicate this in the proposal. Review equipment documentation submitted by the Contractor during normal submittals. From this information develop detailed start -up procedures Consolidate with comments from NYU Hospitals Center Operations and submit to the NYU Hospitals Center Kimmel Program Director for correction and incorporation. Review submitted MEP shop drawings. Ensure the shop drawings conform to the OPR and BOD of the project. Verify maintenance accessibility for equipment. Review piping and ductwork plans for future commissioning w ork. Recommend installation of additional test ports, pressure or temperature gauges, etc. in order to assist the future commissioning effort. Coordinate the commissioning and start up work with the NYU Hospitals Center Kimmel Program Director and Constru ction Manager. Ensure that the commissioning and startup activities are aligned with the overall project schedule. Coordinate with the NYU Hospitals Center Kimmel Program Director on the review of the submitted OEM Predicted Performance Sheets (PPS) for e ach piece of equipment These PPS shall include information on the manufacturer, manufacturer contacts, information on the equipment, process design assumptions, control processes, and process operating information Perform periodic site inspections and wal kthroughs. Review field construction against approved shop submittals. Verify that construction methods and procedures meet design intent and manufacturers recommendations. Verify quality of workmanship meets standards. Document deficiencies in the Construction Deficiencies Report and submit to NYU Hospitals Center Kimmel Program Director. Ensure corrections/rework are performed to the satisfaction of the NYU Hospitals Center Kimmel Program Director. Record comments and deficiencies discovered during walkthroughs and submit to NYU Hospitals Center Kimmel Program Director. Ensure corrections/rework are performed to the satisfaction of the NYU Hospitals Center Kimmel Program Director and the Vice President of Facilities Operations. Observe construction mock-ups of equipment. Ensure field construction matches mock-up for both design conformance (installation and location of valves, etc.) and workmanship. Document deficiencies and submit to the NYU Hospitals Center Kimmel Program Director. Ensure corrections/rework are performed to the satisfaction of the NYU Hospitals Center Kimmel Program Director. Witness all hydrostatic tests of piping. The NYU Hospitals Center requires hydrostatic tests on all fluid containing lines. Refer to the NYU Hospitals Center Standards for pressures and durations. Avoid retesting of lines previously accepted. Document all hydrostatic tests (Contractor forms are acceptable) and include copies in final Commissioning Report. Witness all ductwork pressure tests. The NYU Hospitals Center requires pressure tests on all duct risers and mains, all lab exhausts (including branch ductwork) and all smoke purge life safety ductwork (including branch ductwork). Ensure the allowable leakage rates are not exceeded. Avoid retesting of ducts previously accepted. Document all duct pressure tests (Contractor forms are acceptable) and include copies in final Commissioning Report . Witness and review all systems balancing (Air and Water side). Verify system flows and pressures as required to ensure compliance with design intent and to assure proper operation of connected equipment. Document (Contractor reports are acceptable) all balancing including rework and include copies in final Commissioning Report. Review the start-up plans, equipment and component test procedures with the NYU Hospitals Center Kimmel Program Director, Construction Manager and Operations Personnel to ensure all systems receive proper start-up and commissioning. The Commissioning Authority will notify Owner two weeks in advance of all commissioning activities and testing, which the Owner will have the right participate in and/or witness. Review pre-functional tests and checklists prepared by Cons truction Manager and sub-contractors. Include copies of all pre -functional tests and checklists in final Commissioning Report. Witness the pre-functional testing of equipment to ensure that the equipment is properly installed, and ready for functional per formance testing. Involve the building operation staff in the pre -functional and functional testing. This work includes building maintenance staff as well as the licensed boiler and turbine operating engineers. Coordinate with the NYU Hospitals Center Kimmel Program Director, Engineer -ofRecord, the Contractor, sub -contractors and Operations Perso nnel (the “Team”) on the implementation of the Start-up and testing Plans. Advise Team on the implementation of the plans, and keep the Team informed of any changes to the plans that result from coordination with the Contractor, and sub contractors. Update the commissioning plan and related documentation as required due to changes in equipment or design. The CxA will be responsible for developing these with input from the project Contractor, subcontractors, designers, and equipment manufacturer's representatives. Prior to start-up, gather and review the current control sequences and interlocks and work with the contractors and Engineer -of-Record until sufficient clarity has been obtained, in writing, in order to be able to write detailed testing proce dures. 6. TESTING PHASE General – Plan and conduct meetings to outline the scope of commissioning. This meeting should include the MDG SPM, Start -Up Engineer, sub-contractors, the Engineer of Record, the 's Building Operations personnel, and the Owner. Note that these meetings may be combined with the project meetings. If this is assumed, indicate this in the proposal. Verify all prepared checklists and protocols from Original Equipment Manufacturers and the sub-contractors for start-ups and pre-functional testing of systems. Integrate this documentation into the commissioning plan and include in the final Commissioning Report. Verify with the Engineer of Record and Owner, that all equipment and systems have been properly installed, and that all equipme nt and systems are working in conformance with the requirements of the design and construction documents. Functional Testing of systems and equipment – The CxA shall commission all equipment relating to these systems, ensure the controls system operates as designed and performance test the equipment (as Page 16 of 27 RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING applicable) to provide a complete and functional system. Piping to equipment shall be hydrostatically tested to the last stop valve prior to the equipment connection. In no case are coils to be hydrostatically tested in the field, they shall be valved off prior to the test with blind flanges installed if required . Ductwork pressure tests shall be performed on all duct risers , mains, and exhausts. Controls sequences shall be verified and tested in various operating and failure scenarios. This shall include but not be limited to; normal operation, night setback, unoccupied mode, lead lag operation, power outage, & emergency generator including all operation. Alarms and alarm set-points shall be verified. Control points shall be 100% verified . A full blackout test will be performed, dropping all power feeds from the Utility to ensure that the fire alarms and all systems associated with emergency power perform during both an outage and during a pow er recovery. Failure of any point during this blackout test will require a new blackout test be performed. Testing of emergency power will continue until such time as 100% of all systems and equipment connected to emergency power perform during both outa ge and recovery. The duration of the blackout test will be 36 consecutive hours. The first blackout test will be done just prior to first occupancy (first TCO). Functionally test each piece of equipment and its associated system. The CxA shall assume 100% testing of equipment and devices unless otherwise noted. In order to accomplish this, the CxA shall perform the following: Install sufficient calibrated temperature and humidity sensors and data loggers to trend the temperature and humidity within the building spaces to ensure it does not exceed design criteria Readings should be taken at appropriate intervals (no t to exceed 5 minutes) to assure compliance with design criteria Trend the equipment and system operation through the BMS to track operation of valves and dampers to assure correct operation of the systems Page 20 of 27 RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING Provide simulated heat loads, place on timers to show building diversity and occupied I unoccupied modes. Set BMS to automatic mode and ensure all equipment is in automatic Run the system for 72 hours Upon completion of the test, collect and analyze the data. Provide initial report findings within 48 hours upon the completion of the test . Final report shall follow successful 72 hour test. Any equipment or controls requiring modification during the test shall constitute a failure of the test. The test shall be repeated until no equipment or controls failures occur . Review temperature and humidity levels within all spaces being tested. Document all deviations from acceptable levels or ranges. Fluctuations of significant time or magnitude shall be considered a failure. Review BMS system operation logs for static pressure , water temperatures, differential pressures and other control points . In the event of a test failure, work with Engineer and Controls Vendor to develop corrective actions in order to mitigate issues . Ensure corrective actions are implemented prior to performing a retest. In no circumstance can a test be continued. A new 72 hour test must be scheduled and performed. Upon the successful complet ion of a 72 hour test, a final test report shall be generated. This report shall contain the following : Executive Summary detailing the test(s) performed and results. Ensure testing season is clearly denoted on the report Data from previous unsuccessful tests (if applicable) Corrective actions taken Data from the acceptable test Graphs and Charts as required show trends in the data Confirmation that the systems operate per design Off Season Commissioning After the successful 72 hour test has been completed, return to the site and perform a second 72 hour test of the equipment in the off-season mode (heating if in Winter, or cooling if in Summer). Our intention is to have one 72 hour for a peak heating month and one 72 hour test for a peak cooling m onth. Provide an addendum to the original 72 hour test report with the results. Please provide a fixed cost for a third 72 hour test to be completed upon the request of the NYU Hospitals Center. Page 21 of 27 RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING Training Coordinate and participate in operations and ma intenance staff training. Include any vendors and contractors necessary. The selected firm will be responsible for making a videotape/DVD of the training and submitting it to the Owner to ensure that the training has been satisfactorily completed. Include attendance sheets for personnel training in the commissioning record. Each system shall have a minimum of two (2) training sessions to accommodate the maximum amount of Operations personnel . Three (3) copies of the completed taped training session are required upon completion and should be delivered via a DVD or USB Flash Drive . These shall be included in the final Commissioning Report. Training times may be any time during a 24 hour period to accommodate the off shift of staff. Training goals must be clearly established by the CxA with consultation of the Operations & Maintenance group at the NYU Hospitals Center. These training goals must be met to ensure that staff training was effective. Training shall include classroom and field training. Classroom training shall include review of the approved O&M Manuals, approved as -built drawings, systems description, overall operation modes and various set-points and flows/pressures for equipment. Field training shall include a full walkthrough of the system and all its components including locating all shutoff valves and items requiring maintenance. Controls training shall include review of the actual screens for systems and equipment on the head -end of the Controls Sy stem as well as on local panels, if applicable . Oversee continuing adjustment, optimization, and modification of all plant systems to meet specified operating and regulatory requirements Advise Kimmel Program Director and the Owner on a regular basis on the status of this process. Ensure Contractor provides all required tools, lubricants, miscellaneous start -up consumables and stores them in an acceptable manner until use. Review attic stock requirements and ensure they are provided and stored per manufa cturers recommendations. Verify items are provided per the specification review checklist developed during Design Review . Coordinate with Kimmel Program Director, Engineer of Record, the General Contractor and sub-contractor and all regulatory agencies to make sure all local, State and Federal requirements have been satisfied . Post Commissioning Performance Verification Two months prior to the end of the warranty period, review with the Owner and the building’s operation personnel, the operation of the b uilding and the condition of any Page 22 of 27 RFP FOR COMMISSIONING SERVICES NYU HOSPITALS CENTER KIMMEL PAVILION BUILDING outstanding issues related to the performance of the warranted equipment and systems. The CxA should provide assistance to the building ’s operation personnel in developing reports, documents, and requests for services to ma nufacturers to remedy outstanding warranty problems. Once deficiencies have been corrected, the CxA is to provide oversight on any final testing required and document the results in the final Commissioning Report. Upon completion of the commissioning effort and first 72 hour run test, the CxA shall provide a Final Commissioning Report. This report shall detail the construction and commissioning efforts and their results. The report shall be placed in 3 -Ring binders and be suitable for copying. No pages shall be larger than 8" x 11" Upon review and approval of the report, 4 copies shall be provided to the Owner for its use. The Report shall be broken down as follows and shall include all of the information requested here and elsewhere in this RFP. Table of Contents Executive Summary This section shall detail the major systems and equipment commissioned, and shall provide a summary of the results of the design, construction and commissioning efforts. Commissioning Plan The "as-commissioned" Commission ing Plan shall be included in the Final Commissioning Report Design Report The Design Report shall detail the commissioning efforts during the design phase.
